ABSTRACT: Arch Med Interna 2012 - 34(2): 47-56 Finding a reduced platelet count during pregnancy is always a matter of concern. The multiplicity of etiologies that need to be considered poses a diagnostic challenge for the clinician. This paper gives an overview of the different causes of thrombocytopenia in pregnancy and mentions the rationales for its therapeutic management. In addition to the definition, the paper addresses the potential causes of gestational thrombocytopenia, as well as its relevance and clinical management. The paper then describes the pregnancy-induced complications associated with thrombocytopenia and the rationale for their management. Particular emphasis is made on the hypertensive states of pregnancy, especially preeclampsia and HELLP, as well as thrombotic microangiopathies of pregnancy and the antiphospholipid syndrome. The reciprocal impact of immune thrombocytopenic purpura and gestation is discussed, as well as the relevant aspects of diagnosis and the therapies prescribed, when appropriate. The authors finally describe rare causes of thrombocytopenia during pregnancy.
