Summary No other drugs to date have demonstrated better reductions of cardiovascular disease than statins. This review underlines the continuous evolution of thresholds of LDL-C based on the data of new relevant studies. The actual evidence suggests that the concept of individual cardiovascular risk is likely to dictate the decision on initiating statins treatment rather than simply basing it on individual LDL-C levels. It has also been suggested that there may be no therapeutic threshold for LDL-C reduction beyond which cardiovascular benefit may not be achieved. For secondary prevention intensive therapy is safe and arrests atherosclerosis and provides further clinical benefit with CV risk reduction and hospitalizations for heart failure. In the early phase of acute coronary syndromes (first 10 days), high-doses of statins provide an early and significant reduction in the CV risk. A review of severe side effect of statin underlines their extreme rareness.
