SUMMARY Hypertension is the most frequent chronic disease in adult patients. In the perioperative setting, this is a topic still under revision which has considerably changed, in the last years , the therapeutic as well as the perianesthesia behaviour. A perceptive preoperative evaluation is fundamental to decrease the perioperative risks, to optimize the resources and to reduce costs. The aim of this review is to analyze the present situation in the management of anesthesia to assess the actual impact of hypertension upon the postoperative outcome, and to set some guidelines for the preoperative assessment to those hypertensive patients undergoing noncardiac elective surgery in the Clinical Hospital. A nonsystematic bibliographic search was performed, from the year 1973 to 2009,a total number of ten studies about hypertension and risks of major cardiovascular perioperative complications were analyzed. The association between arterial hypertension and perioperative complications is statistically significant but clinically unimportant. Preoperative evaluation and the elective procedure must be based on the evidence of target-organ damage rather than blood pressure elevations.
