Two cases of whooping cough with high leukocytosis and pulmonary hipertensión who benefited from exchange transfusión. Clinical cases: newborn 27 days and two months old infant who had clinical and laboratory diagnosis of whooping cough. The increasing leukocytosis and the finding of pulmonary hypertension on echocardiogram coupled with the need for mechanical ventilation with high needs necsidad O2 raised the performing exchange transfusion. The technique was performed in both cases smoothly and had a favorable outcome. Conclusion: whooping cough is a problem for young infants not protected by immunization. One complication is the development of high numbers of leukocytes and pulmonary hypertension. The mass reduction of leukocyte through leukoreduction or exchange transfusion are recommended in these cases. We found no previous descriptions of its use in our midst
