Summary  Diabetes mellitus is a chronic non-communicable disease with a high and steadily growing prevalence worldwide. The epidemiological evidence available indicates that it is a major risk for the development of cardiovascular disease (CVD). Chronic hyperglycemia is strongly associated with the occurrence of microvascular complications, and the existing evidence suggests that improving glycemic control results in both short and long-term benefits, preventing not only microvascular, but also macrovascular disease. Attempts to reduce hyperglycemia with its ensuing short and long-term complications have led to the development and introduction of new oral medications and insulins endowed with novel chemical structures and modes of action. Concomitantly, many clinical guidelines and consensus recommendations have been issued to optimize the management of type 2 diabetic patients. Great progress has been made in the understanding of the impact of the various types of sugar-lowering drugs on cardiovascular risk in the last two decades. This paper addresses the relevance of oral agents for the treatment of diabetes, and the evidence available of their association with cardiovascular risk, as well as their positioning in the current clinical guidelines.
