SUMMARY We report the case of a 74 year old female patient carrier of hypertrophic cardiomyopathy with obstructive behavior in whom functional studies and coronary angiography lead to an associated ischemic heart disease diagnosis. We review the main diagnostic criteria of hypertrophic cardiomyopathy in its clinical and echocardiographic aspects, its differential signs compared to valvular aortic stenosis, and its basic pharmacological management, as well as the indication of septal myectomy. Briefly, we summarize the frequency of this association according to the consulted literature.
