Resumo

Summary: secondary prevention frequently is not appropriate due to difficulties in keeping up with lifestyle changes as well as low therapy adherence. Audit permits to know the health care situation, so a self-audit was performed analyzing the process of care (indication of statins, antiplatelets and beta-blockers) and its results (LDL-cholesterol and blood pressure) in patients in secondary prevention. Method: hundred consecutive selected patients with coronary heart disease, cerebrovascular disease and/or arteriopathy were analized, verifying statins, beta blockers and antiplatelets indication, as well as blood pressure and LDL-cholesterol values. Patients with blood pressure <140/90 mmHg (<130/80 mm Hg in diabetics) and LDL-cholesterol <100mg/dl were considered as controlled. Results: eighty-nine percent of patients had registered indication of statins, 98% of antiplatelets and 62% of beta-blockers. Blood pressure was recorded in 100% of the patients and a lipidogram was carried out in 91% of the patients in the last two years. Both parameters were controlled in 36% of the patients, but the figure dropped to 13.3% among diabetics. Patients who had undergone coronary angioplasty were better controlled than the rest (60,9% versus 28,6% p=0,004). Conclusions: though drugs prescription and performed controls were appropriate, only 36% of patients had LDL-cholesterol and blood pressure acceptable values. The self-audit allowed objectively evaluate the control degree in secondary prevention patients, finding most of them poorly controlled, specially the diabetic ones, being necessary to put measures into practice to improve the present situation.
