SUMMARY Background: patients with acute coronary syndrome without ST elevation (ACSWSTE) and absence of significant obstructive coronary lesions have better in hospital prognosis than patients with coronary lesions, but the long term evolution is unknown. Objective: to compare major cardiac events at 5 years in patients with ACSWSTE with and without obstructive coronary lesions and define the characteristics of these populations. Methods: Patients derived to coronary angiography with diagnosis of ACSWSTE were included from 1/1/2003 to 31/7/2004, conforming two groups: without lesions (WL) and with lesions that received angioplasty (L). For the comparison we excluded patients with previous coronary artery disease (myocardial infarction and revascularization). We analyzed the baseline characteristics and cardiovascular risk factors of this population. Follow up included clinical assessment and telephonic questionnaire, evaluating major cardiovascular adverse events (myocardial infarction, new revascularization and death (MACE)). All data was analyzed with SPSS 12.0; the qualitative variables were expressed as percentage, using chi² test and Fischer exact test; for quantitative variables we used t Student test. Results: Of 175 patients WL and 128 patients L, follow up was performed in 150 patients WL and 102 patients L. The mean follow up was 4 years and 9 months, age was 54±11 years in WL and 62±12 years in L (p=0,006), female sex 54,9% in WL and 34,4% in L (p<0,001); smokers 29,7% in WL and 44,5% in L (p<0,001), without statistic differences between the other risk factors. The group WL didn´t have new revascularization, and the group L had 28% (p<0,0001); myocardial infarction occurred in 0,6% in WL and 3,1% in L (p=0,002), death 1,1% in WL and 8,2% in L (p<0,0001), MACE was observed in 1,7% in WL and 39,8% L (p<0,0001). Conclusions: Patients without coronary lesions were predominantly younger, women and less frequently smokers. Patients with ACSWSTE without coronary lesions had better long term prognosis than patients with coronary lesions who underwent coronary angioplasty.
