Breast cancer represents a global public health challenge, being the most diagnosed malignancy in women worldwide according to 2022 GLOBOCAN data. The overall incidence of cancer in Uruguay increases every year, with breast cancer being the most prevalent malignancy and the highest mortality rate among Uruguayan women. Ultra-hypofractionated radiotherapy increases the number of patients treated by reducing the number of days on treatment, promising to reduce overall costs for the healthcare system and ensuring the same oncological outcomes compared to traditional prolonged regimens. The English FAST Forward clinical trial confirmed that the ultra-hypofractionated regimen is effective and safe for breast cancer. Starting in late 2023, the Academic Radiotherapy Unit plans to implement this abbreviated treatment regimen for breast cancer at the university hospital. The first clinical case of a patient with early-stage breast cancer who underwent adjuvant radiotherapy in 5 days is presented. The treatment was well tolerated by the patient. No radiation dermatitis of any grade was observed. In the week following radiation treatment, the patient reported mild fatigue, which subsequently improved. At 6 months of follow-up, the patient is asymptomatic and has no clinical signs of local recurrence.
