Thyroid-associated orbitopathy is an organ-specific autoimmune inflammatory disease affecting the retroocular connective tissue, periorbital muscles, and retroorbital fat. Although it is primarily associated with hyperthyroidism due to Graves’ disease, it can also occur in patients with hypothyroidism secondary to Hashimoto’s thyroiditis, albeit less frequently. We present the case of a patient with hypothyroidism due to Hashimoto’s thyroiditis who developed thyroid orbitopathy. The pathophysiological mechanisms, clinical presentation, and evidence-based therapeutic approach are discussed.
