Se buscaron ensayos clínicos aleatorizados en MEDLINE/PubMed, Embase y Cochrane CENTRAL (hasta diciembre de 2024), incluyendo adultos con várices sin sangrado previo y seguimiento &#8805; 6 meses. Siete estudios cumplieron criterios (seis ECA y una cohorte emparejada; esta última no se metaanalizó); tres ECA aportaron datos homogéneos para síntesis cuantitativa de respuesta hemodinámica (HVPG). Con modelo de efectos aleatorios, no se observaron diferencias significativas entre carvedilol y propranolol en la proporción de respondedores hemodinámicos (diferencia de riesgos 0,01; IC95% &#8722;0,31 a 0,33; p=0,96), con heterogeneidad sustancial (I²=81%; p=0,005). La evidencia directa y consistente para &#8220;primer sangrado&#8221; fue limitada y heterogénea, sin señal concluyente de superioridad clínica de uno u otro fármaco; el perfil de seguridad global fue comparable. El gráfico de embudo no mostró asimetría evidente, si bien su interpretación está restringida por el bajo número de estudios por desenlace. En conjunto, la evidencia disponible de ECA no demuestra superioridad estadísticamente significativa de carvedilol sobre propranolol para profilaxis primaria; se requieren ECA de mayor tamaño y diseño ciego, con potencia adecuada para desenlaces clínicos duros y seguimiento suficiente.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract We conducted a systematic review and meta-analysis, registered in PROSPERO (CRD420251118849) and reported per PRISMA, to compare the efficacy and safety of carvedilol versus propranolol for primary prophylaxis of variceal hemorrhage in cirrhosis. Randomized controlled trials were searched in MEDLINE/PubMed, Embase, and Cochrane CENTRAL through December 2024, including adults with varices without prior bleeding and &#8805;6 months&#8217; follow-up. Seven studies met eligibility (six RCTs and one propensity-matched cohort; the latter was not meta-analyzed); three RCTs provided homogeneous data for quantitative synthesis of hemodynamic response (HVPG). Using a random-effects model, there was no significant difference between carvedilol and propranolol in the proportion of hemodynamic responders (risk difference 0.01; 95% CI &#8722;0.31 to 0.33; p=0.96), with substantial heterogeneity (I²=81%; p=0.005). Direct, consistent evidence on first variceal bleeding was limited and heterogeneous, without a conclusive signal of clinical superiority for either drug; overall safety profiles were comparable. The funnel plot showed no clear asymmetry, although interpretation was limited by the small number of studies per endpoint. Overall, available RCT evidence does not demonstrate statistically significant superiority of carvedilol over propranolol for primary prophylaxis; larger, blinded RCTs with adequate power for hard clinical outcomes and sufficient follow-up are warranted.]]></p></abstract>

Ensaios clínicos randomizados foram pesquisados nas bases MEDLINE/PubMed, Embase e Cochrane CENTRAL até dezembro de 2024, incluindo adultos com varizes sem sangramento prévio e seguimento &#8805;6 meses. Sete estudos preencheram os critérios (seis ECR e uma coorte pareada por escore de propensão; esta última não foi meta-analisada); três ECR forneceram dados homogêneos para síntese quantitativa da resposta hemodinâmica (HVPG). Utilizando modelo de efeitos aleatórios, não houve diferença significativa entre carvedilol e propranolol na proporção de respondedores hemodinâmicos (diferença de risco 0,01; IC95% &#8722;0,31 a 0,33; p=0,96), com heterogeneidade substancial (I²=81%; p=0,005).
