En total, el congreso ha contado con 27 presentaciones cl&iacute;nicas en estas sesiones, 15 actualizaciones de ensayos cl&iacute;nicos, 19 estudios de registro, cuatro nuevas directrices y 4.597 res&uacute;menes. <body><![CDATA[<p style="margin: 0cm 0cm 0.0001pt;"> <span style="font-size: 10pt; font-family: Verdana; ">El objetivo primario fue un compuesto de muerte por causas cardiovasculares u hospitalizaci&oacute;n por IC, y los objetivos secundarios: el tiempo hasta la muerte por cualquier causa, el cambio desde el inicio hasta ocho meses en la puntuaci&oacute;n del cuestionario de miocardiopat&iacute;a de Kansas City (KCCQ) 25 (escala de 0 a 100, puntuaciones m&aacute;s altas indican un menor n&uacute;mero de s&iacute;ntomas y limitaciones f&iacute;sicas asociadas a IC), el tiempo a una nueva aparici&oacute;n de fibrilaci&oacute;n auricular (FA), y el tiempo a la primera aparici&oacute;n de insuficiencia renal (IR)</span><sup><span style="font-size: 10pt; font-family: Verdana; "> (</span><span style="font-size: 10pt; font-family: Verdana; color: rgb(31, 26, 23);"><a href="#2" style="color: blue; text-decoration: underline; text-underline: single">2</a></span><span style="font-size: 10pt; font-family: Verdana; ">,</span><span style="font-size: 10pt; font-family: Verdana; color: rgb(31, 26, 23);"><a href="#3" style="color: blue; text-decoration: underline; text-underline: single">3</a></span><span style="font-size: 10pt; font-family: Verdana; ">)</span></sup><span style="font-size: 10pt; font-family: Verdana; ">.&nbsp; </span><span style="font-size: 10pt; font-family: Verdana;"><o:p></o:p></span></p>           <p style="margin: 0cm 0cm 0.0001pt;"> <span style="font-size: 10pt; font-family: Verdana; ">El estudio fue finalizado antes de tiempo, de acuerdo a las normas preespecificadas, tras un seguimiento de 27 meses, ya que se super&oacute; el l&iacute;mite marcado con un beneficio abrumador con el LCZ696. En el momento del cierre del estudio, se hab&iacute;a dado el objetivo primario en 21,8% de los pacientes en el grupo del LCZ696 y en 26,5% del grupo enalapril (hazard ratio [HR] en el grupo LCZ696, 0,80; intervalo de confianza [CI] 95%: 0,73-0,87; p&lt;0,001). Se registr&oacute; un 17,0% de muertes bajo tratamiento con LCZ696 y 19,8% con enalapril (HR para muerte por cualquier causa, 0,84; CI 95%: 0,76-0,93; p&lt;0,001); de estos pacientes, 13,3% y 16,5%, respectivamente, fueron muertes por causas cardiovasculares (HR 0,80; CI 95%: 0,71-0,89; p&lt;0,001). De los pacientes que recibieron LCZ696, 12,8% fueron hospitalizados por IC, en comparaci&oacute;n con 15,6% de los que recibieron enalapril (HR 0,79; CI 95%: 0,71-0,89, p &lt;0,001)</span><sup><span style="font-size: 10pt; font-family: Verdana; "> (</span><span style="font-size: 10pt; font-family: Verdana; color: rgb(31, 26, 23);"><a href="#2" style="color: blue; text-decoration: underline; text-underline: single">2</a></span><span style="font-size: 10pt; font-family: Verdana; ">)</span></sup><span style="font-size: 10pt; font-family: Verdana; ">.&nbsp; </span><span style="font-size: 10pt; font-family: Verdana;"><o:p></o:p></span></p>           <p style="margin: 0cm 0cm 0.0001pt;"> <span style="font-size: 10pt; font-family: Verdana; ">El cambio medio desde el inicio hasta el mes 8 en la puntuaci&oacute;n KCCQ era una reducci&oacute;n de 2,99 puntos en el grupo LCZ696 y una reducci&oacute;n de 4,63 puntos en el grupo de enalapril (diferencia entre grupos, 1,64 puntos, CI 95%: 0,63-2,65, p=0,001). No hubo diferencias significativas entre ambos grupos en cuanto a la FA de nueva aparici&oacute;n y el desarrollo de IR. El grupo LCZ696 tuvo m&aacute;s proporci&oacute;n de pacientes con hipotensi&oacute;n y angioedema no serio, pero menor proporci&oacute;n de IR, hiperpotasemia y tos que el grupo <span class="GramE">enalapril<sup>(</sup></span></span><sup><span style="font-size: 10pt; font-family: Verdana; color: rgb(31, 26, 23);"><a href="#2" style="color: blue; text-decoration: underline; text-underline: single">2</a></span><span style="font-size: 10pt; font-family: Verdana; ">,</span><span style="font-size: 10pt; font-family: Verdana; color: rgb(31, 26, 23);"><a href="#3" style="color: blue; text-decoration: underline; text-underline: single">3</a></span><span style="font-size: 10pt; font-family: Verdana; ">)</span></sup><span style="font-size: 10pt; font-family: Verdana; ">.&nbsp; </span><span style="font-size: 10pt; font-family: Verdana;"><o:p></o:p></span></p>           <p style="margin: 0cm 0cm 0.0001pt;"> <span style="font-size: 10pt; font-family: Verdana; ">En conclusi&oacute;n, LCZ696 fue superior a enalapril en la reducci&oacute;n del riesgo de muerte y hospitalizaci&oacute;n por IC, con una buena tolerancia. El f&aacute;rmaco fue tambi&eacute;n superior en la reducci&oacute;n del riesgo de muerte por cualquier causa y de los s&iacute;ntomas y limitaciones f&iacute;sicas por IC. Esta experiencia permiti&oacute; prescribir LCZ696 dos veces al d&iacute;a en el estudio PARADIGM-HF, a pesar de su eficacia en monodosis en pacientes con <span class="GramE">hipertensi&oacute;n<sup>(</sup></span></span><sup><span style="font-size: 10pt; font-family: Verdana; color: rgb(31, 26, 23);"><a href="#3" style="color: blue; text-decoration: underline; text-underline: single">3</a></span><span style="font-size: 10pt; font-family: Verdana; ">)</span></sup><span style="font-size: 10pt; font-family: Verdana; ">. La inhibici&oacute;n del bloqueador del receptor de angiotensina/neprilisina combinado podr&iacute;a representar un cambio importante en el tratamiento de pacientes con IC. A pesar de que es sabido que la intervenci&oacute;n coronaria percut&aacute;nea primaria (ICPP) es la mejor estrategia para el tratamiento del s&iacute;ndrome coronario agudo con elevaci&oacute;n del segmento ST (SCACEST), con evidencia de mejores resultados cl&iacute;nicos en comparaci&oacute;n con la terapia fibrinol&iacute;tica, no existe un consenso sobre el mejor tratamiento en la enfermedad coronaria de m&uacute;ltiples vasos detectada en el momento de la <span class="GramE">ICPP<sup><a name="-35"></a>(</sup></span></span><sup><span style="font-size: 10pt; font-family: Verdana; color: rgb(31, 26, 23);"><a href="#35" style="color: blue; text-decoration: underline; text-underline: single">35</a></span><span style="font-size: 10pt; font-family: Verdana; ">)</span></sup><span style="font-size: 10pt; font-family: Verdana; ">. En este contexto, el objetivo de este estudio fue determinar el tratamiento &oacute;ptimo de las lesiones angiogr&aacute;ficamente significativas en arterias coronarias no relacionadas con el infarto (N-IRA) detectadas durante la ICPP. Los criterios de inclusi&oacute;n abarcaron pacientes con SCACEST de hasta 12 horas de evoluci&oacute;n, consentimiento informado, enfermedad multivaso en la coronariograf&iacute;a (estenosis &gt; 70% en una proyecci&oacute;n o &gt; 50% en dos proyecciones), la N-IRA deb&iacute;a ser un vaso grande (&gt; 2 mm) o una rama (&gt; 2 mm) adecuada para el implante de stent. Destac&oacute; que no solo contrastan con los de un metaan&aacute;lisis reciente, sino que tambi&eacute;n contradice la idea dominante sobre el tratamiento de las lesiones N-IRA durante la ICPP, agregando: &ldquo;Hay un acuerdo a ambos lados del Atl&aacute;ntico acerca de que este es un procedimiento potencialmente perjudicial&rdquo;, se&ntilde;alando que dicha pr&aacute;ctica tiene una recomendaci&oacute;n clase III en las gu&iacute;as de tratamiento de la American College of Cardiology/American Heart Asociation y es una acci&oacute;n &ldquo;desaconsejada&rdquo; por la ESC. El Dr. Mehta consider&oacute; dos &ldquo;cuestiones principales&rdquo; en detrimento de CvLPRIT. Por un lado, considera que por ser un estudio peque&ntilde;o tiene un bajo poder estad&iacute;stico y es vulnerable a la intervenci&oacute;n del azar.
