Es una técnica que ha demostrado ser eficaz, con porcentajes de extracción cercanos al 97 %, y segura, con una baja tasa de complicaciones cuando es efectuada con el equipamiento necesario.Presentamos nuestra experiencia inicial en la extracción de diez electrodos,uno de DAI, en cinco pacientes en el Instituto Nacional de Cirugía Cardíaca.]]></p></abstract>

Gu&iacute;as de fijaci&oacute;n (<i>locking stylets</i>), vainas telesc&oacute;picas y la contratracci&oacute;n o contrapulsaci&oacute;n <a name="-1"></a><a name="-2"></a><a name="-3"></a><a name="-4"></a><a name="-5"></a><a name="-6"></a><a name="-7"></a><a name="-8"></a><a name="-9"></a><a name="-10"></a><a name="-11"></a><a name="-12"></a><a name="-13"></a><sup>(<a href="#1">1</a>-<a href="#13">13</a>)</sup>, as&iacute; como cat&eacute;teres con canastillas o &ldquo;<i>loop snares</i>&rdquo; por v&iacute;a femoral <a name="-14"></a><sup>(<a href="#14">14</a>)</sup>, han sustituido a la tracci&oacute;n manual<a name="-15"></a><sup>(<a href="#15">15</a>)</sup>, cuando esta es factible o a la tracci&oacute;n con pesas, m&eacute;todo utilizado en el pasado con pobres resultados. De fallar estos m&eacute;todos deb&iacute;a considerarse la cirug&iacute;a <sup><a name="-16"></a>(<a href="#16">16</a>)</sup>, con toracotom&iacute;a, con el consiguiente aumento de mortalidad, morbilidad y costos. Se est&aacute;n tambi&eacute;n utilizando con &eacute;xito la extracci&oacute;n de electrodos con vainas con l&aacute;ser en el extremo distal de las mismas o vainas con electro cauterio <a name="-17"></a><a name="-18"></a><a name="-19"></a><a name="-20"></a><a name="-21"></a><a name="-22"></a><a name="-23"></a><a name="-24"></a><a name="-25"></a><a name="-26"></a><sup>(<a href="#17">17</a>-<a href="#26">26</a>)</sup>. </font></p>           ]]></body>
<body><![CDATA[<p align="left"><font color="#1f1a17" face="Verdana" size="2">El elemento primordial para las tres t&eacute;cnicas es la gu&iacute;a de fijaci&oacute;n. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">La causa principal para la extracci&oacute;n de los electrodos endocavitarios es la infecci&oacute;n <sup><a name="-27"></a><a name="-28"></a><a name="-29"></a><a name="-30"></a>(<a href="#27">27</a>-<a href="#30">30</a>)</sup>. En escasos pacientes se logr&oacute; la cura s&oacute;lo con tratamiento m&eacute;dico sin necesidad de extraer el sistema implantado <a name="-31"></a><sup>(<a href="#31">31</a>)</sup>. Distintas indicaciones para remover electrodos cr&oacute;nicamente implantados son la interferencia con otros electrodos, sobre todo para los electrodos de los cardiodesfibriladores (DAI), p&eacute;rdida de acceso vascular para nuevos electrodos, y electrodos abandonados que puedan ser de riesgo futuro para el paciente <a name="-32"></a><a name="-33"></a><a name="-34"></a><sup>(<a href="#32">32</a>-<a href="#34">34</a>)</sup>. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">Se han publicado, por parte de NASPE, las indicaciones para la extracci&oacute;n percut&aacute;nea de electrodos, as&iacute; como los requerimientos m&iacute;nimos de recursos humanos y materiales para efectuarlo (tablas <a href="/img/revistas/ruc/v19n1/1a04t2.JPG">2</a>,<a href="/img/revistas/ruc/v19n1/1a04t4.JPG">4</a> y <a href="/img/revistas/ruc/v19n1/1a04t6.JPG">6</a>) <sup>(<a href="#13">13</a>)</sup>. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">En la mayor parte de los casos los electrodos implantados con menos de tres meses pueden ser removidos mediante tracci&oacute;n simple <sup>(<a href="#15">15</a>)</sup>, pero aquellos con m&aacute;s meses o a&ntilde;os desarrollan adherencias fibrosas, que con el tiempo se hacen m&aacute;s densas <a name="-35"></a><sup>(<a href="#35">35</a>)</sup>. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">Pensamos que una de las razones que ha desarrollado esta t&eacute;cnica fue el &ldquo;<i>recall</i>&rdquo; de los electrodos Accufix de Telectronics, aunque luego de a&ntilde;os se observ&oacute; que en la gran mayor&iacute;a de los casos era mayor el riesgo de extraerlos que de dejarlos in situ <a name="-36"></a><a name="-37"></a><sup>(<a href="#36">36</a>,<a href="#37">37</a>)</sup>.</font></p>      <p><font face="Verdana" size="2">    <br>      </font>      </p>     <multicol gutter="18" cols="2"></multicol>     <p align="left"><font color="#1f1a17" face="Verdana" size="2"><b>MATERIAL Y M&Egrave;TODOS</b> </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">Hemos efectuado la extracci&oacute;n de diez electrodos a cinco pacientes con edades que oscilan entre 56 y 78 a&ntilde;os con una media de 68 a&ntilde;os, cuatro de sexo masculino y uno de sexo femenino, con un tiempo de implante de los electrodos que var&iacute;a ente 9 meses y 16 a&ntilde;os. Un electrodo era de cardiodesfibrilador y en la paciente que ten&iacute;a cuatro electrodos uno se extrajo por tracci&oacute;n manual. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">En todos los casos se efectu&oacute; la extracci&oacute;n en la Sala de Marcapasos y Electrofisiolog&iacute;a del Instituto Nacional de Cirug&iacute;a Card&iacute;aca. Todos fueron informados del m&eacute;todo a utilizar y de &nbsp;los eventuales riesgos, obteni&eacute;ndose el consentimiento escrito. Se tomaron rutinas y clasificaci&oacute;n de grupo sangu&iacute;neo. En todos los casos se identific&oacute; la v&iacute;a de abordaje, el tipo de electrodo y el tiempo de implantado (<a href="/img/revistas/ruc/v19n1/1a04t3.JPG">tabla 3</a>). </font></p>           ]]></body>
<body><![CDATA[<p align="left"><font color="#1f1a17" face="Verdana" size="2">En el per&iacute;odo posabsortivo (seis horas de ayuno) se efectu&oacute; monitorizaci&oacute;n de electrocardiograma (ECG), saturaci&oacute;n de O<sub>2</sub> con ox&iacute;metro de pulso y monitorizaci&oacute;n de presi&oacute;n arterial invasiva por punci&oacute;n de arteria radial. En todos los casos se utilizaron parches en posici&oacute;n antero posterior conectados a cardiodesfibrilador Zoll para marcapaseo o desfibrilaci&oacute;n de emergencia, o ambos. En una de las pacientes con bloqueo auriculoventricular completo (BAVC) sin actividad propia se coloc&oacute; adem&aacute;s electrodo Furman para estimulaci&oacute;n transitoria por v&iacute;a femoral derecha. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">Cuatro pacientes requirieron sedaci&oacute;n profunda potenciada con apoyo ventilatorio y un paciente anestesia general con intubaci&oacute;n orotraqueal. En todos los casos estuvo presente con <i>stand-by</i> quir&uacute;rgico real, esto es con equipo y sala prontos en block quir&uacute;rgico ante cualquier eventual complicaci&oacute;n. Todos los pacientes estaban bajo tratamiento con antibi&oacute;ticos de acuerdo al germen aislado o en forma emp&iacute;rica si no se pose&iacute;a este dato. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">En todos los pacientes se utiliz&oacute; el abordaje subclavio. El procedimiento consisti&oacute; en la desinfecci&oacute;n habitual de t&oacute;rax anterior, extracci&oacute;n del generador y liberaci&oacute;n de los electrodos de las adherencias en el bolsillo. Liberaci&oacute;n de puntos de fijaci&oacute;n al plano muscular. Se tomaron muestras para cultivo y antibiograma del bolsillo. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">Con un mandril o gu&iacute;a habitual se comprob&oacute; la indemnidad de la luz interior de la espiral met&aacute;lica (conductor) del electrodo. Se efectu&oacute; medici&oacute;n de la luz interior con calibres adecuados, dado que en los primeros casos se utilizaron gu&iacute;as de sujeci&oacute;n o fijaci&oacute;n marca Cook, modelo Wilkoff, las cuales tienen tres medidas de acuerdo a luz interior de la espiral. En los &uacute;ltimos pacientes se utiliz&oacute; gu&iacute;a marca Cook, modelo Liberator Universal, con una sola medida para todos los electrodos. Se procedi&oacute; a introducirla lo m&aacute;s distal posible, en general llegando al extremo distal del electrodo a traccionar. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">Se efectu&oacute; tracci&oacute;n continua durante 10 a 15 minutos. De no liberarse se introdujo en una primera etapa una vaina telesc&oacute;pica met&aacute;lica para romper adherencias del sector subclavio hasta la desembocadura de la vena cava superior, todo bajo control fluorosc&oacute;pico de alta resoluci&oacute;n con campo con lupa para distinguir bien las estructuras, dado que este es uno de los lugares donde m&aacute;s adherencias existen (<a href="#figura1">figura 1</a>). </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">En caso de no poder liberar el cat&eacute;ter se introdujeron vainas de polietileno o tefl&oacute;n que se avanzaron hasta el extremo distal del electrodo, practicando la contratracci&oacute;n a la masa ventricular (<a href="#figura2">figura 2</a>). </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">Por un lado, mediante la gu&iacute;a estamos traccionando hacia afuera y con la vaina hacia adentro. Esto hace el efecto de contratracci&oacute;n (<a href="#figura3">figura 3</a>). </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">En todos los casos se obtuvo una liberaci&oacute;n total de los electrodos. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">En caso de ser necesario y posible, por ejemplo en caso de obstrucci&oacute;n venosa sin acceso, puede introducirse una gu&iacute;a por la misma vaina que se efectu&oacute; la extracci&oacute;n, introduciendo un nuevo electrodo por los m&eacute;todos convencionales. Dado que todos presentaban infecci&oacute;n, esto no se hizo en ninguno de los casos. En la <a href="/img/revistas/ruc/v19n1/1a04t1.jpg">tabla 1</a> se describen los casos realizados. </font></p>           <p><font face="Verdana" size="2">    ]]></body>
<body><![CDATA[<br>      </font>      </p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2"><b>DISCUSI&Oacute;N</b> </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">La t&eacute;cnica de extracci&oacute;n percut&aacute;nea de electrodos intracavitarios ha demostrado ser eficaz para su utilizaci&oacute;n, con un n&uacute;mero muy bajo de complicaciones <sup>(<a href="#1">1</a>-<a href="#14">14</a>,<a href="#17">17</a>-<a href="#26">26</a>)</sup>. El porcentaje de &eacute;xito var&iacute;a entre 90% y 97% de acuerdo a la experiencia del equipo, el n&uacute;mero de electrodos, el tiempo de implantado, el sexo <sup>(<a href="#1">1</a>-<a href="#4">4</a>,<a href="#8">8</a>-<a href="#10">10</a>)</sup>, y en un porcentaje m&iacute;nimo puede aumentar si se cuenta con equipamiento para poder utilizar l&aacute;ser o electro cauterio <sup>(<a href="#17">17</a>-<a href="#26">26</a>)</sup>. </font></p>     <multicol gutter="18" cols="2"></multicol>     <p align="left"><a href="/img/revistas/ruc/v19n1/1a04t1.jpg"> <font color="#000000" face="Verdana" size="2">TABLA 1</font></a><font color="#1f1a17" face="Verdana" size="2"> </font></p>              <p align="left"><font color="#1f1a17" face="Verdana" size="2">Nosotros no contamos a&uacute;n con ninguna de estas &uacute;ltimas t&eacute;cnicas mencionadas. Describimos nuestra experiencia inicial en la que extrajimos diez electrodos en cinco pacientes con un &eacute;xito total en cuanto a la extracci&oacute;n de los electrodos, sin complicaciones mayores. El porcentaje de complicaciones mayores descrito en la literatura es de 1,9% y menores de 1,5% (<a href="/img/revistas/ruc/v19n1/1a04t5.JPG">tabla 5</a>) <sup>(<a href="#1">1</a>-<a href="#4">4</a>,<a href="#8">8</a>-<a href="#10">10</a>,<a href="#17">17</a>-<a href="#26">26</a>)</sup>. En todos los casos la indicaci&oacute;n fue clase I <sup>(<a href="#13">13</a>)</sup>. En todos hab&iacute;a infecci&oacute;n del bolsillo o endocarditis. El germen mayoritariamente aislado fue estafilococo aureus meticilino sensible, seguido de estafilococo epidermidis, concordando con lo referido en casos de infecci&oacute;n en pacientes con sistemas de estimulaci&oacute;n o DAI implantados <sup>(<a href="#27">27</a>-<a href="#30">30</a>)</sup>. </font></p>     <multicol gutter="18" cols="2"></multicol>     <p align="left"><font color="#1f1a17" face="Verdana" size="2">En aquellos pacientes dependientes de su sistema de estimulaci&oacute;n, se implant&oacute; en el posoperatorio inmediato un sistema de estimulaci&oacute;n epic&aacute;rdico con abordaje subxifoideo (<a href="/img/revistas/ruc/v19n1/1a04f6.jpg">figura 6</a>). </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">En la mayor&iacute;a de los casos para la extracci&oacute;n del electrodo fue suficiente emplear la gu&iacute;a de fijaci&oacute;n con tracci&oacute;n continua y las vainas met&aacute;licas desbridando adherencias de la zona entre la subclavia y la vena cava superior, que es uno de los puntos donde se producen las mayores adherencias. En la literatura el porcentaje de &eacute;xito s&oacute;lo con la tracci&oacute;n continua con las gu&iacute;as de fijaci&oacute;n y desbridamiento proximal var&iacute;a entre 50-70%, debi&eacute;ndose utilizar material adicional (vainas de polietileno) y contrapulsaci&oacute;n en los restantes casos. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">En todos se utiliz&oacute; el abordaje de subclavia con la t&eacute;cnica antes descrita. En casos en que esta falle o en electrodos abandonados sin acceso desde el bolsillo del marcapasos se utiliza el abordaje femoral con tracci&oacute;n y contrapulsaci&oacute;n. Algunos grupos, los menos, utilizan esta t&eacute;cnica como primera opci&oacute;n para electrodos implantados varios a&ntilde;os antes. El grupo de Pisa, de la doctora Bongiorni, describi&oacute; la t&eacute;cnica transyugular, tambi&eacute;n para electrodos abandonados o de dif&iacute;cil extracci&oacute;n mediante el abordaje subclavio. Pensamos, como la mayor&iacute;a, que de tener abordaje subclavio al electrodo, utilizar&iacute;amos como primera opci&oacute;n &eacute;ste, dado que en caso de ser necesario tendr&iacute;amos la posibilidad de colocaci&oacute;n del nuevo sistema de estimulaci&oacute;n, entre otras ventajas. </font></p>           <p><font color="#1f1a17" face="Verdana" size="2">    <br>      </font></p>         ]]></body>

<body><![CDATA[<p align="left"><font color="#1f1a17" face="Verdana" size="2">La extracci&oacute;n percut&aacute;nea de electrodos de marcapasos o DAI endocavitarios puede ser efectuada con un porcentaje de &eacute;xito cercano a 95% y una baja tasa de complicaciones cuando se realiza con todas las precauciones m&iacute;nimas (<i>stand-by</i> quir&uacute;rgico real, material adecuado, equipo de rayos X adecuado y equipo m&eacute;dico entrenado) evit&aacute;ndole al paciente una toracotom&iacute;a con aumento la mortalidad, morbilidad y costos. </font></p>           <p align="left"><font color="#1f1a17" face="Verdana" size="2">Pensamos que con el gran n&uacute;mero de electrodos intracavitarios implantados, estos procedimientos seguramente aumentar&aacute;n con el consiguiente desarrollo de la t&eacute;cnica.
