El objetivo de este estudio es analizar la sobrevida y las variables asociadas con la mortalidad en los pacientes con diagnóstico de ACLF evaluados para trasplante hepático (TH) en la Unidad Bi-Institucional de Trasplante Hepático (Hospital de Clínicas - Hospital Central de las FF.AA.).  Metodología: En este estudio descriptivo, longitudinal, de cohorte histórica, analizamos pacientes que cumplían criterios para ACLF, evaluados para TH entre enero de 2018 y julio de 2023. Se realizó la revisión de la historia clínica electrónica.  Resultados: De los 168 pacientes evaluados para TH, 24% (40) cumplían con criterios de ACLF. La mortalidad global fue 67.5%, con una diferencia estadísticamente significativa entre no trasplantados (82.7%) y trasplantados (27.2%). Al comparar la mortalidad entre los diferentes grados de ACLF (55.5% en ACLF-1, 68.8% en ACLF- 2 y 100% en ACLF-3). La mortalidad fue mayor cuando la causa de descompensación fue una infección bacteriana intercurrente (80%) respecto a otras causas desencadenantes.  Conclusiones: la ACLF es una indicación frecuente de TH, donde la severidad según su grado, la presencia de factores precipitantes como las infecciones bacterianas intercurrentes, y la accesibilidad oportuna al trasplante tienen un impacto significativo en la sobrevida de estos pacientes.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract:  Introduction:  Acute-on-chronic liver failure (ACLF) is a frequent presentation of acute decompensation in cirrhosis, characterised by multiorgan dysfunctions and associated with high mortality. This study aimed to analyse overall survival and the variables associated with mortality in patients diagnosed with ACLF who were evaluated for liver transplantation (LT) at the Bi-Institutional Liver Transplant Unit (Hospital de Clínicas - Hospital Central of the Armed Forces).  Methods:  We conducted a descriptive, longitudinal, historical cohort study of patients meeting ACLF criteria who were assessed for LT between January 2018 and July 2023. Variables were obtained by reviewing the electronic medical record.  Results:  Of 168 patients evaluated for LT, 24 % (n = 40) met ACLF criteria. Overall mortality was 67.5 %, with a statistically significant difference between non-transplanted (82.7 %) and transplanted patients (27.2 %). Mortality by ACLF grade was 55.5 % in ACLF-1, 68.8 % in ACLF-2, and 100 % in ACLF-3. Mortality was higher when the precipitating event was a concurrent bacterial infection (80 %) compared with other triggers.  Conclusions:  ACLF is a common indication for LT. Severity grade, precipitating factors such as concurrent bacterial infections, and timely access to transplantation have a significant impact on patient survival.]]></p></abstract>

Este estudo teve como objetivo analisar a sobrevida e as variáveis associadas à mortalidade em pacientes com diagnóstico de ACLF avaliados para transplante hepático (TH) na Unidade Bi-Institucional de Transplante Hepático (Hospital de Clínicas - Hospital Central das Forças Armadas).  Metodologia:  Trata-se de um estudo descritivo, longitudinal, de coorte histórica, que incluiu pacientes que preenchiam critérios para ACLF e foram avaliados para TH entre janeiro de 2018 e julho de 2023. As variáveis foram obtidas mediante revisão do prontuário eletrônico.  Resultados:  Dos 168 pacientes avaliados para TH, 24 % (n = 40) atendiam aos critérios de ACLF. A mortalidade global foi de 67,5 %, com diferença estatisticamente significativa entre não transplantados (82.7 %) e transplantados (27.2 %). A mortalidade de acordo com o grau de ACLF foi de 55.5 % no ACLF-1, 68.8 % no ACLF-2 e 100 % no ACLF-3.
