Las mujeres representan 20-30% de las cirugías de revascularización miocárdica (CRM) y suelen presentar más resultados adversos, lo cual incluye mayor mortalidad y tasas elevadas de complicaciones posoperatorias como accidente cerebrovascular (ACV) e infarto agudo de miocardio (IAM).  Objetivos:  evaluar resultados a largo plazo de la CRM en mujeres y definir factores predictivos de evolución.  Métodos:  estudio retrospectivo, observacional, aprobado por Comité de Ética. Se analizaron datos del Fondo Nacional de Recursos correspondientes a 21.959 procedimientos de CRM aisladas realizadas entre 2003 y 2022 en Uruguay. Las variables cuantitativas se compararon con test de t y categóricas con Chi cuadrado. Los resultados se ajustaron a variables demográficas mediante regresión logística y de Cox. Se realizaron análisis de subgrupos e interacción con prueba de Mantel-Haenszel para obtener odds ratios (OR). El nivel de significancia estadística se estableció en 0,05.  Resultados:  la mayoría de las variables mostraron diferencias significativas entre sexos. Las mujeres presentaron mayor prevalencia de edad, diabetes, ACV, dislipemia, hipertensión arterial (HTA), lesión de 1 y 2 vasos, fracción de eyección, más ángor inestable y EuroSCORE I más elevado. También tuvieron mayor mortalidad operatoria (4,7% vs. 3,5%; p &lt; 0,001), ACV (1,5% vs. 1,2%; p = 0,02) y IAM (3,2% vs. 2,3%; p &lt; 0,001). La sobrevida ajustada no difirió significativamente entre ambos sexos (HR = 0,96; IC 95%: 0,91-1,01; p = 0,11), la mortalidad operatoria ajustada fue mayor en mujeres (OR = 1,5; IC 95%: 1,27-1,73; p &lt; 0,001).  Conclusiones:  las mujeres presentaron más comorbilidades y mayor riesgo operatorio, con mayor mortalidad y complicaciones. No se hallaron diferencias en mortalidad a largo plazo.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Summary:  Introduction:  ischemic heart disease is the leading cause of death in both sexes, and coronary revascularization is the most common cardiac procedure worldwide. Women, representing 20-30% of coronary artery bypass grafting (CABG) surgeries, tend to experience worse clinical outcomes, including higher mortality and increased severe postoperative complications such as stroke and myocardial infarction (MI).  Objectives:  to evaluate long-term outcomes of CABG in women, identifying clinical predictors of an adverse prognosis.  Methods:  retrospective, observational study. Ethics Committee approved. Data were collected from the National Resource Fund on 21,959 isolated CABG procedures performed in Uruguay between 2003 and 2022. Quantitative variables were compared using t-tests and categorical variables with Chi-square test. Outcomes were adjusted for demographic variables using logistic and Cox regression. Subgroup and interaction analyses were performed using the Mantel-Haenszel test to calculate odds ratios. Statistical significance was set at 0.05.  Results:  most variables showed significant differences between sexes. Women had higher age, diabetes, prior stroke, dyslipidemia, hypertension, 1- and 2-vessel disease, left ventricular ejection fraction, more unstable angina, and higher EuroSCORE I. They also had higher operative mortality (4.7% vs. 3.5%; p &lt; 0.001), stroke (1.5% vs. 1.2%; p = 0.02), and postoperative MI (3.2% vs. 2.3%; p &lt; 0.001). Adjusted survival showed no significant differences (HR = 0.96; 95% CI: 0.91-1.01; p = 0.11). Adjusted operative mortality was higher in women (OR = 1.5; 95% CI: 1.27-1.73; p &lt; 0.001).  Conclusions:  women undergoing CABG had more comorbidities and higher operative risk, with greater operative mortality and postoperative complications. Resultados:  a maioria das variáveis apresentou diferenças significativas entre os sexos. As mulheres tinham maior idade, diabetes, AVC prévio, dislipidemia, hipertensão, doença de 1 e 2 vasos, fração de ejeção, mais angina instável e EuroSCORE I mais alto. Também apresentaram maior mortalidade operatória (4,7% vs. 3,5%; p &lt; 0,001), AVC (1,5% vs. 1,2%; p = 0,02) e IAM (3,2% vs. 2,3%; p &lt; 0,001).
