La magnitud de esos cambios en el perfil de lípidos dependerá del tipo de estatina y de la dosis utilizada; las que son capaces de disminuir un 50% o más el nivel de C-LDL se denominan de alta intensidad. En las últimas décadas, la evidencia proveniente de los grandes ensayos clínicos aleatorizados con estatinas demuestra que la disminución del colesterol reduce la mortalidad cardiovascular, la incidencia de infarto agudo de miocardio y el accidente cerebrovascular. Los primeros ensayos clínicos compararon estatinas de moderada intensidad versus placebo. Posteriormente, algunos estudios evaluaron la hipótesis de que utilizar estatinas de mayor potencia mostraría una mayor reducción de los eventos cardiovasculares en comparación con los esquemas menos intensivos. Asimismo, varias investigaciones han estudiado si el uso de las de alta intensidad impacta en la progresión de la aterosclerosis. En esta revisión se analiza la evidencia que fundamenta las recomendaciones para el uso de estatinas de alta intensidad según las diferentes guías de práctica clínica en prevención primaria y secundaria, identificando los grupos de pacientes que más se favorecen con su indicación. Asimismo, a pesar de las recomendaciones claras de las diferentes guías, se describen los motivos por los cuales su uso en la práctica habitual es deficiente.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Summary: The use of statins to reduce the levels of cholesterol linked to low-density lipoproteins and thus decrease cardiovascular events is one of the most relevant advances in cardiovascular prevention in recent decades. The magnitude of these changes in the lipid profile will depend on the type of statin and the dose used; those that are able to decrease 50% or more low-density lipoproteins levels are called &#8220;high-intensity statins&#8221;. In recent decades, the evidence from large randomized clinical trials with statins shows that lowering cholesterol reduces cardiovascular mortality, the incidence of acute myocardial infarction and cerebrovascular accident. The first clinical trials compared moderate-intensity statins versus placebo. Subsequently, some studies hypothesized that using higher potency statins would show a greater reduction in cardiovascular events compared to less intensive regimens. Also, several trials have studied whether the use of high intensity statins impacts the progression of atherosclerosis. In this review, the evidence that supports the recommendations for the use of high intensity statins according to the different clinical practice guidelines in primary and secondary prevention is analyzed, identifying the groups of patients that are most favored with their indication. Also, despite the clear recommendations of the different guidelines, the reasons why their use in the usual practice is deficient are described.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo: O uso de estatinas para reduzir os níveis de colesterol ligados às lipoproteínas de baixa densidade e, assim, diminuir os eventos cardiovasculares é um dos avanços mais relevantes na prevenção cardiovascular nas últimas décadas. A magnitude dessas alterações no perfil lipídico dependerá do tipo de estatina e da dose utilizada. Aqueles que são capazes de diminuir 50% ou mais do nível das lipoproteínas de baixa densidade são chamadas de &#8220;alta intensidade&#8221;. Nas últimas décadas, as evidências de grandes ensaios clínicos randomizados com estatinas mostram que a redução do colesterol reduz a mortalidade cardiovascular, a incidência de infarto agudo do miocárdio e acidente vascular cerebral. Os primeiros ensaios clínicos compararam estatinas de intensidade moderada versus placebo. Posteriormente, postulou-se que o uso de estatinas de maior potência mostraria uma redução maior nos eventos cardiovasculares em comparação com regimes menos intensivos. Além disso, vários ensaios têm estudado se o uso de estatinas de alta intensidade afeta a progressão da aterosclerose. Nesta revisão, analisamos as evidências que corroboram as recomendações para o uso de estatinas de alta intensidade de acordo com as diferentes diretrizes de prática clínica na prevenção primária e secundária, identificando os grupos de pacientes mais favorecidos com sua indicação. Além disso, apesar das recomendações claras das diferentes diretrizes, foram descritas as razões pelas quais seu uso na prática usual é deficiente.]]></p></abstract>
