Desde entonces muchos dispositivos se han creado a tales efectos, teniendo la bioingeniería médica un gran desafío en encontrar el dispositivo ideal. En total se ha intentado el cierre de la comunicación interauricular tipo ostium secundum en 30 pacientes, con edades entre 16 meses y 35 años, con peso mínimo de 8,5 kg. En tres casos se utilizó un dispositivo Cardioseal y en los 27 casos restantes el dispositivo utilizado fue el ASO Amplatzer. En tres casos el cierre no fue posible debido a la variedad anatómica del defecto, siendo extraídos antes de ser liberados sin complicaciones. En los 27 casos restantes se logró el cierre del defecto en forma total, en un caso se registró una insuficiencia mitral mínima que desapareció a los tres meses. En todos los casos se utilizó en forma simultánea el ecocardiograma Doppler color transesofágico. A todos los pacientes se les otorgó el alta a domicilio promedialmente a las 18 horas. Se efectuó cobertura antibiótica pre, intra y postprocedimiento. Todos quedaron con antiagregantes plaquetarios (ácido acetilsalicílico) durante seis meses. En suma, la oclusión de la comunicación interauricular por medio de dispositivos oclusores implantados mediante cateterismo intervencionista es un procedimiento sencillo, práctico y con excelentes resultados, con breve período de internación y menor agresión psico-física para el paciente comparado con la cirugía.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[SUMMARY Since King and Miller&rsquo;s publication in 1974, it has been demonstrated that certain atrial septal defects can be closed by percutaneous technique. Many devices have been developed with that purpose. It has been a major challenge for medical bioengineers to design the ideal one. A total of 30 patients with ostium secundum type atrial septal defects were attempted to close (ages 16 months to 35 years, minimum weight 8,5 kg.). A Cardioseal device was utilized in 3 cases, an Amplatzer in the other 27. In 3 patients the percutaneous closure was not possible because of anatomical features. Both devices were removed without complications, before their release. In 27 patients, the defect was completely closed, there was one case with residual mild mitral insufficiency that disappeared in 3 months. In all of the procedures, simultaneous transesophageal echocardiography was utilized. The patients were discharged home a medium of 18 hours after the procedure. They received peri-procedural antibiotic coverage. Antiplatelet treatment with aspirin was maintained for 6 months. In conclusion, atrial septal defects closure with percutaneous implantable devices is a simple and practical procedure, with excellent results, and much less stressful hospital experience in comparison with heart surgery.]]></p></abstract>

<body><![CDATA[<br>        &nbsp;&nbsp;&nbsp;&nbsp;HEART CATHETERIZATION </font></p>     <p align="left">&nbsp;</p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2"><b>    <br>        INTRODUCCI&Oacute;N</b> </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Las comunicaciones interauriculares aisladas tipo ostium secundum sin asociaci&oacute;n con otra cardiopat&iacute;a que tenga sanci&oacute;n quir&uacute;rgica (<a href="#figura1">figura 1</a>), son aquellas pasibles de ser cerradas por dispositivo oclusor implantado mediante cateterismo card&iacute;aco <a name="-1"></a><a name="-2"></a><a name="-3"></a><a name="-4"></a><sup>(<a href="#1">1</a>-<a href="#4">4</a>)</sup>. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">La indicaci&oacute;n de efectuar el cierre de dichos defectos radica en la existencia de un cortocircuito de izquierda a derecha con dilataci&oacute;n de cavidades derechas o la existencia de embolia paradojal <a name="-5"></a><a name="-6"></a><sup>(<a href="#5">5</a>,<a href="#6">6</a>)</sup>. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Desde los trabajos de King y Mills en 1974 <sup><a name="-7"></a><a name="-8"></a>(<a href="#7">7</a>,<a href="#8">8</a>)</sup> y Lock en 1989 <a name="-9"></a><sup>(<a href="#9">9</a>)</sup> se ha demostrado que el cierre de determinadas comunicaciones interauriculares es posible de ser efectuado mediante cateterismo card&iacute;aco con total eficacia, con menor tiempo de internaci&oacute;n, ausencia de toracotom&iacute;a, menor agresi&oacute;n psicof&iacute;sica para el paciente, y ser un procedimiento m&aacute;s eficiente en la ecuaci&oacute;n costo/resultado <sup><a name="-10"></a><a name="-11"></a><a name="-12"></a><a name="-13"></a><a name="-14"></a>(<a href="#10">10</a>-<a href="#14">14</a>)</sup>. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">En este trabajo se presenta la experiencia del Instituto de Cardiolog&iacute;a Infantil (ICI) en el cierre transcut&aacute;neo de la comunicaci&oacute;n interauricular iniciada en el a&ntilde;o 1997.</font></p>                     <p align="left"><font color="#1f1a17" face="Verdana" size="2"><b>MATERIAL Y M&eacute;TODO</b> </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">En nuestro servicio iniciamos la experiencia en los a&ntilde;os 1997-1998, reinici&aacute;ndola de manera sistem&aacute;tica en el a&ntilde;o 2003 al haber aprobado el Fondo Nacional de Recursos (FNR) la financiaci&oacute;n para la utilizaci&oacute;n en exclusividad de los dispositivos ASO Amplatzer. </font></p>         <font color="#1f1a17" face="Verdana" size="2">Se ha intentado el cierre de la comunicaci&oacute;n interauricular (CIA) en 30 pacientes portadores de una comunicaci&oacute;n interauricular tipo ostium secundum (<a href="#figura2">figura 2</a>), asociando en dos casos una deformaci&oacute;n aneurism&aacute;tica del s&eacute;ptum interatrial (<a href="#figura3">figura 3</a>), en dos casos se trataba de un defecto m&uacute;ltiple del s&eacute;ptum interatrial con cortocircuito izquierda&ndash;derecha mayor de 1,5:1 con dilataci&oacute;n y sobrecarga de volumen de cavidades derechas que fue cerrado con el implante de un solo dispositivo y en un caso exist&iacute;a una seudomembrana que tabicaba parcialmente el defecto. </font>        <font face="Verdana" size="2">            ]]></body>
<body><![CDATA[<br> &nbsp;                          <br> </font> <font color="#1f1a17" face="Verdana" size="2">Los criterios de indicaci&oacute;n de cierre mediante dispositivo son: CIA tipo ostium secundum con cortocircuito izquierda-derecha, dilataci&oacute;n de cavidades derechas y bordes adecuados para el correcto anclaje del dispositivo como se describe en la <a href="#tabla1">tabla 1</a>. Las contraindicaciones para realizar dicho procedimiento se representan en la <a href="/img/revistas/ruc/v19n2-3/2-3a02t2.JPG">tabla 2</a>. </font>            <p align="left"><font color="#1f1a17" face="Verdana" size="2">De los 30 pacientes, 15 eran del sexo femenino y 15 del sexo masculino. Las edades oscilaron entre 16 meses y 35 a&ntilde;os. El peso m&iacute;nimo fue de 8,5 kilogramos (<a href="/img/revistas/ruc/v19n2-3/2-3a02t3.JPG">tabla 3</a>). </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Todos los pacientes llegaron a la sala de hemodinamia con un ayuno m&iacute;nimo de cuatro horas, dependiendo de la edad, y con una v&iacute;a venosa perif&eacute;rica para la administraci&oacute;n de sueros e iniciar la medicaci&oacute;n anest&eacute;sica. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Se control&oacute; previamente y durante todo el procedimiento el electrocardiograma en forma continua, la saturaci&oacute;n arterial de ox&iacute;geno mediante ox&iacute;metro de pulso, la presi&oacute;n arterial medida por m&eacute;todo no invasivo y la temperatura rectal en los pacientes menores de 3 a&ntilde;os. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">En todos los casos se realiz&oacute; premedicaci&oacute;n anest&eacute;sica con midazolam y fentanylo, con posterior intubaci&oacute;n orotraqueal y ventilaci&oacute;n asistida, continuando la anestesia con sevorane al 2%<a name="-15"></a><sup>(<a href="#15">15</a>)</sup>. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Se coloc&oacute; sonda intraesof&aacute;gica pedi&aacute;trica multiplanar y multifrecuencia para la realizaci&oacute;n del ecocardiograma transesof&aacute;gico (ETE) con equipo General Electric Vivid 7. Se efectu&oacute; topograf&iacute;a, medici&oacute;n del defecto y de los bordes para corroborar la indicaci&oacute;n del cierre con dispositivo <sup><a name="-16"></a>(<a href="#16">16</a>)</sup>.</font></p>          <p align="left"><font face="Verdana" size="2">    <br>   </font>   </p>          <p align="left"><font face="Verdana" size="2"><a name="tabla1"></a><img style="width: 417px; height: 189px;" alt="" src="/img/revistas/ruc/v19n2-3/2-3a02t1.JPG">&nbsp;</font></p>      <font face="Verdana" size="2">      <a href="/img/revistas/ruc/v19n2-3/2-3a02t2.JPG">Tabla 2</a>    ]]></body>
<body><![CDATA[<br>          <br>          <br>         </font>             <p><font face="Verdana" size="2">    <br>       &nbsp;<a href="/img/revistas/ruc/v19n2-3/2-3a02t3.JPG">Tabla 3</a> </font> </p>       <multicol gutter="18" cols="2"></multicol>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Se realiza cateterismo v&iacute;a vena femoral con medici&oacute;n de presiones y oximetr&iacute;as para c&aacute;lculo de cortocircuito y relaci&oacute;n flujo pulmonar-flujo sist&eacute;mico (QP/QS). Luego se efect&uacute;a una angiocardiograf&iacute;a utilizando un equipo biplano Phillips Integris BH 3000, inyectando el medio de contraste (Hexabrix 320) en vena pulmonar superior derecha con proyecci&oacute;n oblicua anterior izquierda y angulaci&oacute;n craneal para topografiar el defecto y realizar su medici&oacute;n (figuras <a href="#figura4">4</a> y <a href="#figura5">5</a>). Se realiza heparinizaci&oacute;n sist&eacute;mica a raz&oacute;n de 100 unidades de heparina s&oacute;dica por kilogramo de peso. </font></p>               <p><font color="#1f1a17" face="Verdana" size="2">Se emplea la t&eacute;cnica descrita para cierre de la CIA y se procede a realizar la medici&oacute;n del tama&ntilde;o del defecto con el insuflado del bal&oacute;n a trav&eacute;s del ecocardiograma transesof&aacute;gico y la imagen fluorosc&oacute;pica del mismo (<a href="#figura6">figura 6</a>). El procedimiento se repite, si es necesario, para reconfirmar la medici&oacute;n, punto crucial a fin de determinar el tama&ntilde;o del dispositivo a emplear <a name="-17"></a><a name="-18"></a><a name="-19"></a><sup>(<a href="#17">17</a>-<a href="#19">19</a>)</sup>. Una vez obtenida la medida exacta del defecto se elige el dispositivo, tipo y tama&ntilde;o del mismo.</font><font face="Verdana" size="2">    <br>        </font>        </p>       <multicol gutter="18" cols="2"></multicol>     <p align="left"><font color="#1f1a17" face="Verdana" size="2">En nuestra serie se utilizaron tres dispositivos Cardioseal y 29 dispositivos Amplatzer en 27 pacientes <a name="-20"></a><a name="-21"></a><a name="-22"></a><a name="-23"></a><sup>(<a href="#20">20</a>-<a href="#23">23</a>)</sup>. Se posiciona el dispositivo a nivel del defecto interatrial (<a href="#figura7">figura 7</a>). Al corroborar por ETE y radioscop&iacute;a su correcta ubicaci&oacute;n y ausencia de compromiso de estructuras nobles vecinas se lo libera (figuras <a href="#figura8">8</a> y <a href="#figura9">9</a>). </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Se realiza profilaxis antibi&oacute;tica. </font></p>             ]]></body>
<body><![CDATA[<p align="left"><font color="#1f1a17" face="Verdana" size="2">Se retira la v&iacute;a venosa femoral con control de hemostasis local por compresi&oacute;n digital. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Se despierta al paciente y se lo extuba. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Se pasa a sala con control de par&aacute;metros vitales y del sitio de punci&oacute;n. Una vez reinstalada la v&iacute;a oral se inician los antiagregantes plaquetarios que se mantendr&aacute;n por seis meses. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Se otorg&oacute; el alta a domicilio en todos los casos con un promedio de 18 horas de internaci&oacute;n luego de efectuado el procedimiento. </font></p>             <p><font face="Verdana" size="2">    <br>        </font>        </p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2"><b>RESULTADOS</b> </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Nuestros resultados se presentan en la <a href="/img/revistas/ruc/v19n2-3/2-3a02t3.JPG">tabla 3</a>. Se observ&oacute; que en 27 de 30 pacientes (90%) portadores de CIA tipo ostium secundum que se consideraron para cierre con dispositivo, se obtuvo el cierre adecuado. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Es un hecho habitual en este tipo de procedimiento la persistencia de un m&iacute;nimo cortocircuito a trav&eacute;s del dispositivo inmediatamente de implantado que desaparece totalmente en las primeras horas. En nuestra serie en 22 pacientes no se document&oacute; ning&uacute;n cortocircuito por ETE a la salida de la sala de hemodinamia, en cinco pacientes se document&oacute; un cortocircuito m&iacute;nimo a trav&eacute;s del dispositivo que desaparece totalmente antes del alta en valoraci&oacute;n por ecocardiograma Doppler color transtor&aacute;cico (<a href="/img/revistas/ruc/v19n2-3/2-3a02t4.JPG">tabla 4</a>). </font></p>             <p><font face="Verdana" size="2">    ]]></body>

<body><![CDATA[<br>        </font></p>      <font color="#1f1a17" face="Verdana" size="2">Un paciente presentaba un s&iacute;ndrome de Marfan con moderada dilataci&oacute;n de la ra&iacute;z a&oacute;rtica e insuficiencia a&oacute;rtica m&iacute;nima sin indicaci&oacute;n de intervenci&oacute;n quir&uacute;rgica en el momento actual, y adem&aacute;s una CIA del tipo ostium secundum con moderado cortocircuito izquierda-derecha con indicaci&oacute;n de proceder a su cierre, donde se impon&iacute;a tratar de efectuar el cierre por dispositivo mediante cateterismo, disminuyendo el n&uacute;mero de eventuales cirug&iacute;as que pueda requerir el paciente en el futuro. El paciente se deriva a cirug&iacute;a para su cierre, donde se comprueba la presencia de un sector del s&eacute;ptum de tama&ntilde;o adecuado pero con una consistencia muy laxa que no permit&iacute;a un correcto sost&eacute;n del dispositivo. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">En un tercer paciente se posicion&oacute; en forma correcta el dispositivo, de tama&ntilde;o adecuado para los di&aacute;metros del defecto, no logr&aacute;ndose un correcto anclaje, siendo desplazado hacia la aur&iacute;cula izquierda al empujar el mismo con su cat&eacute;ter gu&iacute;a, por lo que se lo envaina y retira previo a su liberaci&oacute;n, requiriendo un dispositivo de mayor tama&ntilde;o que no se pudo utilizar debido a que el di&aacute;metro total del s&eacute;ptum interatrial no lo permit&iacute;a. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">En estos tres casos el dispositivo que se intent&oacute; implantar fue el ASO de Amplatzer. </font></p>             ]]></body>
<body><![CDATA[<p align="left"><font color="#1f1a17" face="Verdana" size="2">Como complicaci&oacute;n se observ&oacute; en un caso una insuficiencia mitral de grado m&iacute;nimo que desapareci&oacute; en forma espont&aacute;nea a los tres &nbsp;meses de realizado el procedimiento, hecho documentado por ETE. </font></p>             <p><font face="Verdana" size="2">    <br>        </font>        </p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2"><b>DISCUSI&oacute;N</b> </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">La comunicaci&oacute;n interauricular puede presentarse sola o asociada a otras alteraciones cong&eacute;nitas card&iacute;acas, las cuales determinar&aacute;n la conducta a seguir y el pron&oacute;stico del paciente. Su ubicaci&oacute;n puede estar a diferentes niveles en el s&eacute;ptum interauricular. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">La indicaci&oacute;n de cierre de las mismas mediante dispositivo oclusor implantado por cateterismo card&iacute;aco comprende a aquellos pacientes que presentan una CIA tipo ostium secundum con cortocircuito izquierda-derecha y bordes adecuados <sup><a name="-24"></a><a name="-25"></a>(<a href="#24">24</a>,<a href="#25">25</a>)</sup>. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">En nuestra casu&iacute;stica se utilizaron tres dispositivos ASO Cardioseal que fueron implantados en la primera serie, dejando de utilizarse posteriormente por las ventajas t&eacute;cnicas que ofrece el dispositivo ASO Amplatzer y porque, adem&aacute;s, es el &uacute;nico dispositivo autorizado y financiado por el FNR en el momento actual. </font></p>               <p><font color="#1f1a17" face="Verdana" size="2">Con los dispositivos existentes en la actualidad se puede proceder al cierre de las comunicaciones interauriculares que tienen indicaci&oacute;n para el mismo con un margen adecuado de seguridad, sabiendo que existen riesgos potenciales que deben ser evitados (<a href="#tabla5">tabla 5</a>) <sup><a name="-26"></a><a name="-27"></a><a name="-28"></a><a name="-29"></a>(<a href="#26">26</a>-<a href="#29">29</a>)</sup>. Sin embargo, con un manejo adecuado del procedimiento, con todo el instrumental necesario y con el trabajo en conjunto de los equipos de hemodinamia y ecograf&iacute;a se logra evitar que aqu&eacute;llos sucedan.</font><font face="Verdana" size="2">    <br>       &nbsp; </font> </p>       <multicol gutter="18" cols="2"></multicol>     <p align="left"><font color="#1f1a17" face="Verdana" size="2">De acuerdo a la literatura internacional nuestros resultados son comparables en cuanto a la efectividad del cierre del defecto, con ausencia de complicaciones, siendo la incidencia de &eacute;stas muy baja seg&uacute;n se refiere <a name="-30"></a><sup>(<a href="#30">30</a>)</sup>. </font></p>             ]]></body>
<body><![CDATA[<p align="left"><font color="#1f1a17" face="Verdana" size="2">Cabe destacar que los trabajos existentes en la literatura revelan la normalizaci&oacute;n de las dimensiones del ventr&iacute;culo derecho tras la oclusi&oacute;n del defecto septal interatrial a mediano plazo <a name="-31"></a><a name="-32"></a><sup>(<a href="#31">31</a>,<a href="#32">32</a>)</sup>. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">Cabe destacar que en nuestro pa&iacute;s el costo total del procedimiento de cierre por cateterismo card&iacute;aco de este tipo de defectos es inferior al arancel de cirug&iacute;a card&iacute;aca pedi&aacute;trica establecido por el organismo financiador. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">En conclusi&oacute;n, frente a la presencia de una CIA que tiene indicaci&oacute;n de cierre se debe valorar la eventualidad de proceder a ocluirla mediante dispositivo implantado por cateterismo card&iacute;aco, donde el cierre eficaz se obtiene con menor morbilidad, sin cicatrices tor&aacute;cicas, m&iacute;nimo per&iacute;odo de internaci&oacute;n y menor agresi&oacute;n psicof&iacute;sica. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">El procedimiento implica la estrecha colaboraci&oacute;n entre el equipo de ecocardiograf&iacute;a con el equipo de hemodinamia ya que el &eacute;xito del resultado depende de un adecuado ensamble entre las decisiones que se deben adoptar por uno y otro para seleccionar tanto el dispositivo como su tama&ntilde;o y valorar, antes de su liberaci&oacute;n, la seguridad de que no se produzca su migraci&oacute;n, as&iacute; como prevenir el potencial compromiso de estructuras nobles vecinas. </font></p>             <p align="left"><font color="#1f1a17" face="Verdana" size="2">A 30 a&ntilde;os de la publicaci&oacute;n del primer cierre de una CIA mediante cateterismo intervencionista, muchos dispositivos se han creado y otros tantos han ca&iacute;do en desuso debido a los problemas que planteaban, disponi&eacute;ndose en la actualidad de dispositivos &ldquo;ideales&rdquo; a tales efectos.
