Los carcinomas uroteliales músculo-invasores tienen una probabilidad &#8805;50% de desarrollar metástasis durante su evolución. En las últimas tres décadas, el tratamiento del cáncer de vejiga metastásico ha sido la quimioterapia basada en platinos. Aunque inicialmente efectiva, no logra generar respuestas duraderas, y la supervivencia mediana ronda los 15 meses. En los últimos años, se han presentado varios trabajos pivotales que han cambiado el estándar de tratamiento del cáncer de vejiga metastásico mediante el empleo de inmunoterapia, terapias dirigidas, conjugados anticuerpo-fármaco y combinaciones de estos. En el Congreso de la Sociedad Europea de Oncología Médica (ESMO) 2023, se presentaron dos estudios que revolucionaron el tratamiento del cáncer urotelial metastásico. Estos trabajos, publicados en PubMed (período 2020-abril 2024), generaron cambios en las guías de tratamiento oncológico a nivel global. Este artículo revisa críticamente dichos avances, destacando su impacto en la supervivencia y calidad de vida de los pacientes, así como su potencial para redefinir el manejo terapéutico de la enfermedad.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract Bladder cancer is the most common malignant neoplasm of the urinary system, with a global incidence of half a million cases per year. Muscle-invasive urothelial carcinomas have a &#8805;50% chance of developing metastasis during their progression. In the last three decades, the treatment of metastatic bladder cancer has been platinum-based chemotherapy. Although initially effective, it fails to produce durable responses, and the median survival is around 15 months. In recent years, several pivotal studies have changed the standard treatment for metastatic bladder cancer through the use of immunotherapy, targeted therapies, antibody-drug conjugates, and combinations of these. At the 2023 European Society for Medical Oncology (ESMO) Congress, two studies were presented that revolutionized the treatment of metastatic urothelial cancer. These studies, published in PubMed (period 2020-April 2024), have led to changes in global oncological treatment guidelines. This article critically reviews these advances, highlighting their impact on patient survival and quality of life, as well as their potential to redefine the therapeutic management of the disease.]]></p></abstract>

Nas últimas três décadas, o tratamento do câncer de bexiga metastático tem sido a quimioterapia baseada em platina. Embora inicialmente eficaz, ela não gera respostas duradouras, e a sobrevida mediana gira em torno de 15 meses. Nos últimos anos, vários estudos importantes mudaram o padrão de tratamento do câncer de bexiga metastático, por meio do uso de imunoterapia, terapias direcionadas, conjugados anticorpo-fármaco e combinações destes.
