SARS-COV-2 genera compromiso predominantemente pulmonar, aunque también se ha observado una infección sistémica que afecta el sistema hematopoyético y la coagulación. A pesar de los notables avances en su comprensión, el virus continúa activo y resulta de gran interés práctico contar con parámetros biológicos que permitan alertarnos sobre una posible evolución desfavorable.  Metodología:  Estudio descriptivo, prospectivo, no intervencionista, de las alteraciones hematológicas e inflamatorias en pacientes ingresados por COVID-19 en Hospital Británico, Montevideo, Uruguay.  Resultados:  Se incluyeron 209 pacientes, 126 (60%) hombres, con mediana de edad de 59 años (24-98). Cuarenta y dos pacientes (20.1%) requirieron atención en Unidad de Cuidada Intensivo de Adultos (UCIA), con una mediana de edad de 65 años (36-87). Valores de LDH &gt; 250 U/L se relacionaron con mayor ingreso a UCIA (p. 0.001), así como ferritina &gt; 1000 ng/ml y PCR &gt; 60 mg/L, (p= 0.002 y p= 0.001 respectivamente). El 28.57% de los pacientes ingresados a UCIA falleció. EL 76.19% de los pacientes ingresados a UCIA presentaron linfopenia (&lt; 1000), (p=0.005). Un Ratio Neutrófilo/Linfocito (RNL) alto se asoció con mayor probabilidad de muerte (OR=1.041, IC 1.008 - 1.075, p 0.015). La comorbilidad con mayor letalidad fue la HTA (OR= 3.637, IC 95% 1.358 - 9.739, p 0.010).  Conclusiones:  La edad &gt; 65 años, linfopenia &lt; 1000, el aumento de parámetros inflamatorios se asoció con mayor ingreso a UCIA. HTA, DD &gt; 1000 mcg/L, LDH &gt; 250 U/L y PCR &gt; 60 mg/L se asociaron con mayor mortalidad en general, en pacientes con COVID-19.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract:  Introduction:  In 2020, the World Health Organization (WHO) declared the coronavirus disease (COVID-19) a pandemic. The SARS-CoV-2 virus predominantly affects the lungs but has also been observed to cause systemic infection involving the hematopoietic system and coagulation. Despite significant advances in understanding the virus, it persists actively, making it practically important to identify biological parameters that can alert us to a potential unfavorable progression.  Methods: Descriptive, prospective, non-interventional study of hematological and inflammatory alterations in patients admitted for COVID-19 at Hospital Británico, Montevideo, Uruguay.  Results:  Two hundred and nine patients were included, 126 (60%) of whom were men, with a median age of 59 years (24-98). Forty-two patients (20.1%) required care in the Adult Intensive Care Unit (AICU), with a median age of 65 years (36-87). LDH values &gt; 250 U/L were associated with higher AICU admission (p=0.001), as well as ferritin &gt; 1000 ng/ml and CRP &gt; 60 mg/L (p=0.002 and p=0.001, respectively). Of the patients admitted to AICU, 28.57% died. Additionally, 76.19% of AICU patients presented with lymphopenia (&lt; 1000), (p=0.005). A high Neutrophil-to-Lymphocyte Ratio (NLR) was linked to a higher probability of death (OR=1.041, CI 1.008-1.075, p=0.015). Apesar dos avanços significativos na compreensão do vírus, ele continua ativo e é de grande interesse prático ter parâmetros biológicos que possam nos alertar sobre uma possível evolução desfavorável.  Metodologia:  Estudo descritivo, prospectivo e não intervencionista das alterações hematológicas e inflamatórias em pacientes internados por COVID-19 no Hospital Britânico, Montevidéu, Uruguai.  Resultados:  Foram incluídos 209 pacientes, 126 (60%) homens, com mediana de idade de 59 anos (24-98). Quarenta e dois pacientes (20,1%) necessitaram de cuidados na Unidade de Terapia Intensiva (UTI) Adulto, com mediana de idade de 65 anos (36-87). Valores de LDH &gt; 250 U/L foram associados a maior admissão em UTI (p. 0,001), assim como ferritina &gt; 1000 ng/ml e PCR &gt; 60 mg/L, (p= 0,002 e p= 0,001 respectivamente). 28,57% dos pacientes internados na UTI morreram. 76,19% dos pacientes internados na UTI apresentaram linfopenia (&lt;1000), (p=0,005). Uma alta relação neutrófilos/linfócitos (NLR) foi associada a uma maior probabilidade de morte (OR=1,041, IC 1,008 - 1,075, p 0,015). A comorbidade com maior taxa de mortalidade foi a hipertensão arterial (OR= 3,637, IC 95% 1,358 - 9,739, p 0,010).
