Alrededor del 6 a 10% de los BIT con componente intra torácico según algunas series, se requiere abordaje transtorácico para garantizar una resección completa y segura. Material y métodos: Se realizó un estudio descriptivo que incluyó un paciente con diagnóstico de bocio intratorácico compresivo intervenido quirúrgicamente mediante un abordaje con cervicotomía en &#8220;T&#8221; invertida. Se complementa con una revisión narrativa de la literatura científica disponible sobre indicaciones y técnica quirúrgica en el tratamiento de bocio de gran volumen. Resultados: Paciente de sexo masculino, 57 años. Consulta por disnea, estridor laríngeo y disfagia para sólidos. Se diagnostica bocio con gran masa intratorácica. Se aborda por incisión en cuello y esternotomía en &#8220;T&#8221; sin complicaciones. Objetivo: describir los predictores de acceso transtorácico y detalles técnicos quirúrgicos del abordaje mediante esternotomía del bocio intra torácico. La tomografía computada es el estudio diagnóstico de referencia para definir la necesidad de realizar un abordaje accesorio a la cervicotomía. La esternotomía en T invertida permite una amplia exposición del mediastino y una disección segura de las estructuras contiguas.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract Intrathoracic goiter (ITG) is defined as a growth of the thyroid gland that extends into the thorax and can be located completely or partially in the mediastinum. Approximately 6 to 10% of ITGs with an intrathoracic component, according to some series, require a transthoracic approach to ensure complete and safe resection. Materials and methods: A descriptive study was conducted that included a patient diagnosed with compressive intrathoracic goiter who underwent surgery using an inverted "T" cervicotomy approach. It was complemented by a narrative review of the available scientific literature on indications and surgical technique for the treatment of large-volume goiters. Results: A 57-year-old male patient presented with dyspnea, laryngeal stridor, and dysphagia for solids. A goiter with a large intrathoracic mass was diagnosed. The procedure was approached via a neck incision and a "T" sternotomy, without complications. Objective: To describe the predictors of transthoracic access and the technical details of the sternotomy approach to intrathoracic goiter. Computed tomography is the gold standard for determining the need for an accessory approach to cervicotomy. The inverted T sternotomy allows for wide exposure of the mediastinum and safe dissection of adjacent structures.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo Bócio intratorácico (GIT) é definido como o crescimento da glândula tireoide que se estende até o tórax e pode estar localizado total ou parcialmente no mediastino. Em aproximadamente 6 a 10% dos ITBs com componente intratorácico, segundo algumas séries, é necessária uma abordagem transtorácica para garantir uma ressecção completa e segura. Materiais e métodos: Foi realizado um estudo descritivo que incluiu um paciente com diagnóstico de bócio intratorácico compressivo submetido à cirurgia por meio de cervicotomia em &#8220;T&#8221; invertido. É complementado por uma revisão narrativa da literatura científica disponível sobre indicações e técnica cirúrgica no tratamento do bócio de grande volume. Resultados: Paciente do sexo masculino, 57 anos. Consulta para dispneia, estridor laríngeo e disfagia para sólidos. Foi diagnosticado um bócio com uma grande massa intratorácica. É abordado por incisão no pescoço e esternotomia em &#8220;T&#8221; sem complicações. Objetivo: Descrever os preditores do acesso transtorácico e os detalhes técnicos cirúrgicos da abordagem por esternotomia para bócio intratorácico. A tomografia computadorizada é o estudo diagnóstico de referência para definir a necessidade de uma abordagem acessória à cervicotomia. A esternotomia em T invertido permite ampla exposição do mediastino e dissecção segura das estruturas adjacentes.]]></p></abstract>
