Superada la etapa crítica, fecuentemente presentan dificultades para iniciar la alimentación: rechazo hasta aversión por la comida y saciedad precoz. Esto se podría interpretar como un mecanismo &#8220;símil dolor neuropático&#8221; secundario a los reiterados estímulos orales dolorosos y displacenteros de los procedimientos invasivos en boca y cuello. La literatura propone que una prueba terapéutica con fármacos indicados para el tratamiento del dolor neuropático, como amitriptilina o gabapentina, podría contribuir a la resolución de dicho problema.  Caso clínico:  niño de 1 año 9 meses, portador de síndrome de Di George, cardiopatía congénita compleja por la que fue sometido a cirugía en dos oportunidades, desnutrición crónica severa, hipoparatiroidismo, hipogamaglobulinemia con linfopenia y traqueostomía. Presentó aversión persistente para la alimentación oral y fue tratado con gabapentina oral con buena respuesta clínica: aceptación progresiva de la alimentación por boca que permitió retiro de la sonda nasogástrica y ganancia sostenida de peso.  Discusión: las experiencias dolorosas secundarias a tratamientos invasivos realizados a niños con cardiopatías congénitas y otras EALV, podrían determinar sensibilización de nociceptores periféricos, activación y excitación del sistema nervioso central e hiperalgesia con el consecuente disconfort oral, odinofagia, disfagia, saciedad precoz y rechazo de la comida. La gabapentina podría ser considerada un fármaco efectivo para inhibir dichos mecanismos, previo a definir medidas intervencionistas, como una gastrostomía, que agregan mayor complejidad en el abordaje de los niños.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Summary:  Introduction:  many children with life-threatening conditions (LTC) require, in their first months of life, prolonged stays in intensive care. Once the critical stage has passed, they frequently present difficulties in initiating feeding: food rejection or aversion and early satiety. This could be interpreted as a &#8220;neuropathic pain-like&#8221; mechanism secondary to the repeated painful and unpleasant oral stimuli of invasive procedures in the mouth and neck. The literature proposes that a therapeutic trial with drugs indicated for the treatment of neuropathic pain such as amitriptyline or gabapentin could contribute to the resolution of this problem.  Clinical case:  child, 1 year 9 months old, with Di George syndrome, complex congenital heart disease for which he underwent surgery on two occasions, severe chronic malnutrition, hypoparathyroidism, hypogammaglobulinemia with lymphopenia and tracheostomy. He had a persistent aversion to oral feeding and was treated with oral gabapentin with good clinical response: progressive acceptance of oral feeding that allowed removal of the nasogastric tube and sustained weight gain.  Discussion: painful experiences secondary to invasive treatments performed on children with congenital heart disease and other LTC could determine sensitization of peripheral nociceptors, activation and excitation of the central nervous system and hyperalgesia with consequent oral discomfort, odynophagia, dysphagia, early satiety and rejection of meal. Gabapentin could be considered an effective drug to inhibit these mechanisms, prior to defining interventional measures such as a gastrostomy, which add greater complexity to the approach to children.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo:  Introdução:  muitas crianças com doenças potencialmente fatais (DPF) necessitam, nos primeiros meses de vida, de estadias prolongadas em cuidados intensivos. Passada a fase crítica, frequentemente apresentam dificuldades no início da alimentação: rejeição à aversão à comida e saciedade precoce. Isto poderia ser interpretado como um mecanismo &#8220;semelhante à dor neuropática&#8221; secundário aos repetidos estímulos orais dolorosos e desagradáveis de procedimentos invasivos na boca e pescoço. A literatura propõe que um ensaio terapêutico com medicamentos indicados para o tratamento da dor neuropática, como a amitriptilina ou a gabapentina, poderia contribuir para a resolução deste problema.  Caso clínico: criança, 1 ano e 9 meses, portadora de síndrome de Di George, cardiopatia congênita complexa, submetida a duas cirurgias, desnutrição crônica grave, hipoparatireoidismo, hipogamaglobulinemia com linfopenia e traqueostomia. Apresentava aversão persistente à alimentação oral e foi tratado com gabapentina oral com boa resposta clínica: aceitação progressiva da alimentação oral que permitiu a retirada da sonda nasogástrica e ganho de peso sustentado.  Discussão: experiências dolorosas secundárias a tratamentos invasivos realizados em crianças com cardiopatias congênitas e outras DPF poderiam determinar sensibilização de nociceptores periféricos, ativação e excitação do sistema nervoso central e hiperalgesia com consequente desconforto oral, odinofagia, disfagia, saciedade precoce e rejeição de refeição. A gabapentina poderia ser considerada um medicamento eficaz para inibir esses mecanismos, antes da definição de medidas intervencionistas como a gastrostomia, que agregam maior complexidade à abordagem das crianças.]]></p></abstract>
