La comunicación interventricular (CIV) post-IAM ocurre como consecuencia de la necrosis del tejido miocárdico, que lleva a su debilitamiento y rotura, lo que genera una solución de continuidad entre ambos ventrículos. La inestabilidad hemodinámica, la insuficiencia cardíaca y/o la aparición de un nuevo soplo constituyen la forma de presentación más frecuente. El diagnóstico se basa en los hallazgos del ecocardiograma transtorácico (ETT) como principal herramienta, aunque a veces puede resultar desafiante debido a la complejidad y anfractuosidad de este tipo de defectos. El tratamiento de elección es quirúrgico, sin embargo, en ocasiones es difícil determinar su oportunidad; la conducta habitual es diferir esta intervención hasta lograr cierta cicatrización del tejido miocárdico que permita obtener mejores resultados. Se presenta un caso clínico que ilustra la evolución ecocardiográfica de una CIV post-IAM, desde el diagnóstico inicial hasta la intervención quirúrgica.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract Mechanical complications of acute myocardial infarction (AMI) are relatively uncommon but carry significant clinical relevance due to their high associated mortality. Post-infarction ventricular septal rupture (VSR) results from myocardial tissue necrosis, leading to structural weakening and eventual disruption of the interventricular septum, thereby creating a pathological communication between the left and right ventricles. The most frequent clinical presentations include hemodynamic instability, acute heart failure, and/or the emergence of a new cardiac murmur. The diagnosis is primarily established through transthoracic echocardiography (TTE), which serves as the main diagnostic modality. However, identification can be challenging due to the anatomical complexity and irregular morphology of these septal defects. Surgical repair remains the treatment of choice, although determining the optimal timing of intervention is often complex. It is generally recommended to delay surgery until some degree of myocardial scarring has occurred, in order to improve surgical outcomes. This article presents a clinical case illustrating the echocardiographic progression of a post-infarction VSR from initial diagnosis through to definitive surgical intervention.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo As complicações mecânicas do infarto agudo do miocárdio (IAM) são eventos relativamente raros, porém de extrema relevância clínica, em virtude de sua elevada taxa de mortalidade. A comunicação interventricular (CIV) pós-IAM representa uma consequência da necrose do tecido miocárdico, que leva ao seu enfraquecimento estrutural e subsequente ruptura, o que resulta em uma descontinuidade entre os ventrículos esquerdo e direito. As manifestações clínicas mais comuns incluem instabilidade hemodinâmica, insuficiência cardíaca aguda e/ou o surgimento de um novo sopro cardíaco. O diagnóstico é estabelecido, principalmente, por meio do ecocardiograma transtorácico (ETT), que constitui a principal ferramenta de avaliação inicial. No entanto, a identificação pode ser desafiadora em virtude da complexidade anatômica e da morfologia irregular desses defeitos septais. O tratamento de escolha é a correção cirúrgica, embora a definição do momento ideal para a intervenção nem sempre seja simples. Em geral, recomenda-se o adiamento da cirurgia até que ocorra um grau mínimo de cicatrização do tecido miocárdico, com o objetivo de otimizar os resultados cirúrgicos. Apresenta-se, neste artigo, um caso clínico ilustrativo que descreve a evolução ecocardiográfica de uma CIV pós-infarto, desde o diagnóstico inicial até a abordagem cirúrgica definitiva.]]></p></abstract>
