El cuestionario PROMIS®-disnea, que evalúa el síntoma disnea y su severidad, ha sido extensamente validado y sugerido para su aplicación en ensayos clínicos. Sin embargo, su uso en el escenario de la cardiotoxicidad no ha sido descrito. Objetivo: explorar la potencial utilidad del cuestionario PROMIS®-disnea para detectar insuficiencia cardíaca o disfunción sistodiastólica en sujetos con linfoma bajo tratamiento quimioterápico que participan de un programa de ejercicio. Método: se incluyeron pacientes ingresados al ensayo clínico AEROHEMONCO (NCT04476576) que finalizaron la quimioterapia y cumplieron un programa de ejercicio individualizado de 12 semanas. Se aplicó PROMIS®-disnea al inicio, a los 2 y 3 meses y al finalizar el programa de ejercicio. Se correlacionaron los valores de PROMIS®-disnea con variables ecocardiográficas, clínicas y de laboratorio. Se calculó la incidencia acumulada de cardiotoxicidad. Resultados: 19 sujetos iniciaron quimioterapia por linfoma; 12 ingresaron a AEROHEMONCO y 9 completaron el programa de ejercicio. Los puntajes del cuestionario PROMIS®-disnea muestran variaciones entre las etapas de medición. La única correlación significativa es con el valor de hemoglobina (Spearman rho: - 0,43, p = 0,02). La incidencia acumulada de cardiotoxicidad a los 4 meses fue de 0,22. Conclusiones: en pacientes con linfoma sometidos a quimioterapia y un programa de ejercicio, los valores de PROMIS®-disnea presentan variaciones durante el seguimiento, y podrían estar influenciados por la presencia y severidad de la anemia. Se requiere ampliar la serie de pacientes para definir su aplicabilidad para identificar la cardiotoxicidad en este contexto clínico-terapéutico.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Summary: Introduction: the cardiotoxic effect of anthracyclines is defined by the presence of clinical heart failure and/or decreased left ventricular systolic function associated with its use. The PROMIS®-dyspnea questionnaire, which assesses the symptom of dyspnea and its severity, has been extensively validated and suggested for use in clinical trials. However, its use in the cardiotoxicity scenario has not been described. Objective: to explore the potential usefulness of the PROMIS®-dyspnea questionnaire to detect heart failure or systo-diastolic dysfunction in subjects with lymphoma undergoing chemotherapy who participate in a physical exercise program. Method: subjects included in the AEROHEMONCO clinical trial (NCT04476576) who completed the chemotherapy plan and 12 weeks of individualized exercise program were included. The PROMIS®-dyspnea questionnaire was administered at the beginning, at 2 and 3 months and at the end of the physical exercise program. Correlation was made between PROMIS®-dyspnea and echocardiographic, clinical, and laboratory variables. Cumulative incidence of cardiotoxicity was calculated. Results: 19 subjects started chemotherapy for lymphoma; 12 entered AEROHEMONCO and 9 completed the physical exercise program. The questionnaire shows significant variations between measurement stages. The only significant correlation is with hemoglobin value (Spearman rho: - 0.43, p = 0.02). The cumulative incidence of cardiotoxicity in 4 months was 0.22. Conclusions: in patients with lymphoma undergoing chemotherapy and an exercise program, PROMIS®-dyspnea values vary during follow-up, and may be influenced by the presence and severity of anemia. It is necessary to expand the patient series to define its applicability in order to identify cardiotoxicity in this therapeutic clinical context.]]></p></abstract>

O questionário PROMIS®-dispneia, que avalia o sintoma de dispneia e sua gravidade, foi amplamente validado e sugerido para aplicação em ensaios clínicos. No entanto, o seu uso no cenário de cardiotoxicidade não foi descrito. Objetivo: explorar a potencial utilidade do questionário PROMIS®-dispneia para detectar insuficiência cardíaca ou disfunção sisto-diastólica em indivíduos com linfoma em tratamento quimioterápico que participam de um programa de exercícios físicos. Método: indivíduos incluídos no ensaio clínico AEROHEMONCO (NCT04476576) que completaram o plano de quimioterapia e completaram 12 semanas de programa de exercícios físicos individualizados foram incluídos. O questionário PROMIS®-dispneia foi aplicado no início, aos 2 e 3 meses e ao final do programa de exercícios físicos. Foi feita correlação entre PROMIS®-dispneia e variáveis ecocardiográficas, clínicas e laboratoriais. A incidência cumulativa de cardiotoxicidade foi calculada.
