La genotipificación del &#8220;gen RHD&#8221; fetal en sangre materna, realizada con un kit de desarrollo nacional, permite identificar díadas materno-fetales de igual grupo, lo que podría evitar la profilaxis con inmunoglobulina anti-D. Se trató de un estudio multicéntrico prospectivo, descriptivo, de cohorte única de pacientes, de 92 gestantes RhD negativas, no aloinmunizadas, cursando embarazos con feto único. Se extrajo una muestra de sangre periférica entre las semanas 17 y 28 de gestación. Se utilizó el kit de detección molecular del gen RHD del Laboratorio ATGen, que consiste en un PCR en tiempo real en formato multiplex con sondas TaqMan, permitiendo amplificación simultánea y diferencial de tres regiones del gen RHD (exones 5, 7 y 10). Se compararon los resultados con la determinación RhD serológica en sangre de cordón neonatal (método de referencia). De 92 muestras, 5 fueron descartadas por no cumplir con los criterios de inclusión del presente estudio (2 correspondientes a madres RhD positivas y 3 por problemas preanalíticos). Cincuenta y siete muestras fueron positivas para los tres exones evaluados, con valores de Ct concordantes y cumpliendo los criterios de adecuación definidos en el inserto del kit (Cts&#8804;37). De estas 57 muestras maternas, el neonato fue positivo mediante serología del antígeno RhD postnacimiento. Las muestras negativas para el gen RHD fetal (n=30) fueron negativas para los tres exones del kit, y se confirmó que, en sangre de cordón, el antígeno RhD era negativo. El kit molecular de producción nacional identificó los tres exones fetales en plasma materno, con sensibilidad y especificidad de 100%, incorporando en Uruguay esta nueva tecnología.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract The Uruguayan regulation requires serological RhD determination for all maternal-neonatal dyads. Fetal RHD genotyping in maternal blood, performed with a nationally developed kit, allows for the identification of same-group maternal-fetal dyads, which could potentially prevent anti-D immunoglobulin prophylaxis. This was a prospective, descriptive, single-cohort, multicenter study of 92 RhD-negative, non-alloimmunized pregnant women with a single-fetus pregnancy. A peripheral blood sample was collected between weeks 17 and 28 of gestation. An ATGen Laboratory kit was used via real-time multiplex PCR with TaqMan probes, allowing for simultaneous and differential amplification of three regions of the RHD gene (exons 5, 7 and 10). The results were compared with serological RhD determination in neonatal cord blood. Out of 92 samples, 5 were discarded (2 from RhD-positive mothers and 3 with pre-analytical issues). Fifty-seven samples were positive for the three evaluated exons, with concordant Ct values and meeting the adequacy criteria defined in the insert (Cts&#8804;37). For these 57 maternal samples, their neonate was positive via post-birth serological RhD antigen testing. The samples negative for the fetal RHD gene (n=30) were negative for the 3 exons of the kit, and it was confirmed that the RhD antigen in the cord blood was negative. The nationally produced molecular kit identified the three fetal exons in maternal plasma with 100% sensitivity and specificity, incorporating this new technology in Uruguay.]]></p></abstract>

A genotipagem do gene RHD fetal em sangue materno, realizada com um kit de desenvolvimento nacional, permite identificar díades materno-fetais com o mesmo grupo RhD, o que poderia evitar a profilaxia com imunoglobulina anti-D. Tratou-se de um estudo multicêntrico, prospetivo, descritivo, de coorte única, incluindo 92 gestantes RhD negativas, não aloimunizadas, com gestações de feto único. Foi coletada uma amostra de sangue periférico entre as semanas 17 e 28 de gestação. Utilizou-se o kit de deteção molecular do gene RHD do Laboratório ATGen, que consiste em uma PCR em tempo real em formato multiplex com sondas TaqMan, permitindo a amplificação simultânea e diferencial de três regiões do gene RHD (éxons 5, 7 e 10). Os resultados foram comparados com a determinação sorológica do RhD em sangue de cordão umbilical neonatal (método de referência). Das 92 amostras, 5 foram excluídas por não atenderem aos critérios de inclusão do presente estudo (2 correspondentes a mães RhD positivas e 3 devido a problemas pré-analíticos). Cinquenta e sete amostras foram positivas para os três éxons avaliados, com valores de Ct concordantes e atendendo aos critérios de adequação definidos no inserto do kit (Ct &#8804; 37). Nessas 57 amostras maternas, o neonato apresentou sorologia positiva para o antígeno RhD no período pós-natal. As amostras negativas para o gene RHD fetal (n = 30) foram negativas para os três éxons do kit, e confirmou-se que, no sangue de cordão umbilical, o antígeno RhD era negativo.
