We present the case of a patient who came to the clinic with chronic fatigue, fragile skin, increased abdominal girth, lower limb edema, and insomnia. Laboratory studies were requested, revealing decreased serum cortisol and ACTH levels. During the second follow-up visit, the patient was referred to the chronic use of steroid cream and ingestion due to pharyngeal discomfort. The cream was discontinued after being replaced with an oral steroid in a tapering regimen, leading to improvement in the patient's condition. This article reviews the literature and describes the patient's progress. It is important to emphasize that patients often do not disclose the medications they usually take or their routes of administration during the consultation.
