Goodpasture syndrome is a small-vessel vasculitis mediated by autoantibodies directed against type IV collagen, which is found in the glomerular and alveolar basement membranes. Clinically, it presents as a pulmonary-renal syndrome, characterized by diffuse alveolar hemorrhage and rapidly progressive glomerulonephritis.

We report the case of a 61-year-old woman with hemoptysis, severe anemia, and acute kidney injury, in whom radiologic findings were consistent with alveolar hemorrhage and high titers of anti-glomerular basement membrane antibodies were documented. Despite treatment with corticosteroids, plasmapheresis, and cyclophosphamide, the clinical course was unfavorable.

This case illustrates the aggressiveness of Goodpasture syndrome in older adults, highlighting the importance of early diagnosis, timely initiation of immunosuppressive therapy, and multidisciplinary management.
