Hypertriglyceridemia is considered severe when the patient presents values between 500-880 mg/dL and extreme when values are greater than 880 mg/dL. The causes of extreme HTG can be classified as primary-or genetic-and secondary, such as a high-fat, high-glycemic-index diet, alcohol consumption, obesity, diabetes mellitus, hypothyroidism, kidney disease, and medications. The treatment of asymptomatic HTG is under constant review, and the indication for therapeutic plasma exchange remains controversial. Two clinical cases of patients with extreme hypertriglyceridemia are presented, with a review of the literature focusing on therapeutic aspects.
