Thromboangiitis obliterans (TAO) is a segmental, non-atherosclerotic inflammatory disease that affects small and medium-sized arteries, veins and nerves of the extremities. Smoking is the main pathogenetic factor. We present the clinical case of a young man, a smoker, who presented dry necrosis of the 1st, 2nd and 3rd phalanx of the left hand. Arterial Doppler ultrasound showed occluded left radial and ulnar arteries. After excluding other etiologies and with compatible histopathological findings, a diagnosis of TAO was made. This disease is rare and should be suspected in young, male, smoker patients with a history of intermittent claudication, ulcer or gangrene. Diagnosis is clinical and by exclusion. Smoking cessation is the cornerstone of treatment.
