Spontaneous rupture of the extensor carpi ulnaris is a rare injury that has a significant impact on the biomechanics of the wrist, limiting movements such as supination and ulnar deviation. A 63-year-old man presented with pain in the dorsal region of the right forearm and difficulty moving the wrist. Imaging revealed a rupture of the extensor carpi ulnaris tendon, and he underwent surgery with a palmaris longus tendon graft, with adequate progress and recovery of range of motion in the wrist joint. Tendon transfer is an effective option when conventional repair options are not viable.
