ABSTRACT: Arch Med Interna 2014 - 36(2):96-98Introduction. Dandy-Walker’s malformation has been classically described through the triad including cystic dilation of the fourth ventricle, hypoplasia of the cerebellar vermis and hydrocephalus. Symptoms typically occur in early childhood. Presentation during adulthood is extremely rare, although it may appear either spontaneously or following head trauma. Clinical case: 24-year-old male that seeks care at the emergency room complaining of symptoms consistent with intracranial hypertension. The head CT scan revealed the Dandy-Walker malformation. Comments and conclusions. Approximately 80% of the patients with this malformation present within their first year of life, primarily with an enlarged skull secondary to hydrocephalus. Conversely, in the adult the condition usually presents with symptoms related with the dysfunction of hind structures that cause nystagmus, gait changes and ataxia, although they can also present with symptoms of increased intracranial pressure.
