Atherosclerotic cardiovascular disease remains the number one cause of mortality and morbidity in the world. Traditional approach to atherosclerosis disease based on the identification and treatment of risk factors has failed. Detection of high risk subclinical atherosclerosis has been omitted. Therefore this traditional approach is not valid to define cardiovascular risk strategies. As the artery is the main protagonist of atherosclerosis disease becomes necessary to evaluate it directly by structural and functional testing. We must improve the traditional, imprecise risk factor approach using a new method based on non invasive screening for the disease itself. The early detection of subclinical atherosclerosis defines an accurate landmark to identify cardiovascular risk (individual cardiovascular risk).This better approach has the potential to change the field of preventive cardiology
