The aim of this study was to demonstrate the efficacy and safety of the infraclavicular approach of the brachial plexus blockade for the analgesia and anaesthesia of the superior limb, in a hospital environment without echography. It was done in 63 patients with no contraindications , which were coordinated for arm, forearm, elbow ,wrist and hand surgery. The approach was at the infraclavicular fosa, by vertical punction, looking for distal muscular response by neurolocalization at 0,5 mA. 30-40 ml of lidocaine 1% and bupyvacaine 0,375% and 0,25% was utilized. We obtained muscular response in 59 patients, distal response in 58, flexion of the forearm in one patient, paresthesia without muscular response in two patients, no response in two patients. We seek for anaesthesia in 24 patients, having success in 21, the remaining three do have analgesia. In all cases we seek success analgesia, having EVA between 0-1 in the recovery room. Had no pneumothorax or elements of local anesthetics intoxication, three venipuncture that did not prevent the blockade and a neuroapraxia with complete resolution in a month. This approach is effective and safe for the realization of anesthesia and analgesia for upper limb, for that we believe it should be a tool for more widespread use
