SUMMARY In neurosurgery, the ordinary pre-anesthetic evaluation must be done according to the surgical pathology, owing to some cases are associated to specific anesthetic problems, like difficult intubation, cardiac disease and endocrine alterations. The present review deals with those cases according to the following scheme: sellar and para-sellar lesions (pituitary tumors and craniopharyngiomas), craniofacial deformations (craniosynostosis) and spina bifida. This scheme doesn’t include the neurosurgical diseases that not deserve a different pre-anesthetic evaluation apart from the standard done for all patients. For each disease, we’ve exposed the diagnostic algorithm of the anatomic and physiologic alterations that could generate anesthetic problems, together to the perioperative therapeutic management to provide help to the anesthesiologist in charge.
