SUMMARY A short update concerning Unexpected Cardiac Arrest during Spinal Anesthesia is presented. The complication is uncommon but, as it takes place in young and healthy patients, it can be defined as an anesthesic catastrophe. The realtions among vasodilatation, hypovolemia with a reduction of the venous filling of the heart, and other not well explained reflexes, are emphasized as the physiopatologic basis of the problem. A vigilant attitude, specially when working on young and /or vagotonic, and/or athletic population, seems to be the basis for prevention. The multimodal approach from the very beginning of the problem, based on vagolytic and vasoconstrictor drugs, is the clue for the treatment. A question is raised concerning in which is the timing for sending the patient to the ward, taking into account that several cases have been described after a lapse of time.
