Summary: Introduction. Central venous lines (CVL’s) are important tools in current medicine and different approaches are described. One of the most used is the posterior approach of the internal jugular vein with the Brinkman - Costley technique. Currently, the medial approach of this vein through the Sedillot triangle has gained more acceptance among our’s anesthesiologists. Objective. To evaluate the efficacy on the cannulation of the internal jugular vein using two approaches: posterior and medial. Frequency of complications was also determined. Method and Material. This is a prospective and comparative study; the choice of the approach was decided by the physician involved in the procedure, based on preference and experience. The success or failure of cannulation of the vein was recorded, as well as number of attempts. Results. 62 CVL’s were evaluated, 34 with the medial approach and 28 with the posterior. The rate of success with the medial approach was 88,2% compared with 64,2% with the posterior (p=0.034). 93,3% of the CVL’s were obtained at the first attempt contrasting with 61,1% with the posterior approach (p=0.009). Arterial puncture was the only complication observed.   Conclusions. The medial approach was more effective than the posterior approach, with a similar rate of complications.
