Background. The timing principle is an accepted technique to obtain an artificial airway quickly, using non depolarizing neuromuscular blocking agents. Rocuronium has a rapid onset of action which may lead to use it as the relaxant of choice for this objective. Conditions of tracheal intubation were evaluated comparing timing dose of rocuronium and crush induction with succinilcoline. Methods. This prospective study included 2 groups of patients, ASA physical status 1 and 2, female and male, between 15 - 63 years old, undergoing elective surgery. Patients were allocated into two groups: Group A (n = 9) = rocuronium 0,6 mg/kg 20 seconds before hipnotic injection (timing dose); Group B (n = 9) = succinylcholine 1,5 mg/kg immediately after hipnotic administration (fast endotracheal intubation). Each patient received thiopental 4 mg/kg as hipnotic agent, and neuromuscular transmission was monitored with TOF-Guard which use an accelerometer to meseaure. Intubating conditions were assessed according to a grading scale. In addition, the time since hypnotic injection until 10% of neuromuscular response (t 1) and until tracheal intubation (t2 ), was obtained. Results. Intubating conditions were excellent or good in both groups. The data obtained (mean ± SD) was: 1) Group A: t 1 22,6 ± 12,1 seconds and t2 42,4 ±14,8 seconds; 2) Group B: t 1 fue de 39,2 ± 19 seconds and t2 54 ± 8,9 seconds. There was a statistical difference between the groups for t2, p<0,01. None of the patients expressed muscular weakness before hypnosis. Conclusions. The timing principle with rocuronium seems to be a clinical option that gives appropriate intubating conditions (short time and excellent conditions). All patients were satisfied with the technique, without any disconfort.
