Health care for young lesbian, gay, bisexual and transgender youth (LGBT) is becoming an emergent need. The Health Program for Adolescents of the Department of Health of the Sao Paulo state, in collaboration with PAHO has been reflecting on this issue and allowing the implementation of a service network for young LGBT. This work describes the opening up of a space for reflection and debate on homophobia and other topics in connection with this public. The methodology for a participative implementation of guidelines for the comprehensive care for the LGBT youth is described, bearing in mind the opinions shared in the groups that operate in the Adolescents House of Sao Paulo and the public opinion surveys on health during the “LGBT Pride Parade” in 2008 and 2013.
