Abstract

Sumary    Portal hypertension (P H) is an uncommon condition in children but potentially severe when it occurs due to gastrointestinal bleeding esophageal varices with a mortality of up to 25%. Diseases that cause PH often occurs with a silent clinical expression at the beginning hindering timely diagnosis. Objective: to determine the presentation and causes PH in patients admitted to a pediatric service polyvalent. Methodology: retrospective analysis of the medical records of patients admitted to the pediatric department of the Sanatorio Americano between January 2007 and December 2010 with diagnosis of portal hypertension, liver disease, hepatitis or gastrointestinal pathology. Results: 3.581 children were admitted, 173 presented the above diagnoses. Portal hypertension was confirmed in 6 patients. The pathology that caused  P H were autoimmune hepatitis, alpha 1 antitrypsin deficiency, and portal vein thrombosis secondary to umbilical catheter in the neonatal period. All had splenomegaly, four esophageal varices and one of them ascites. Two children presented upper gastrointestinal bleeding. The time from onset of symptoms to diagnosis of PH was variable according to the cause of PH. Conclusions: coinciding with the international literature PH is a disease of low incidence being found causes similar to those reported worldwide. Being PH a complication of a variety of diseases, it is important that the general pediatrician became acquainted with their clinical presentations in order to arrive at an early diagnosis allowing prompt treatment to decrease the morbidity and mortality of this condition.
