Summary Loxosceles is a spider of global distribution. It is responsible for skin necrosis that in a low percentage is accompanied by systemic manifestations, its mortality can be high. Diagnosis is associated with localized painful injuries or by identification of the spider. Recommendation for the use of serum antiloxosceles is not uniform. In this article we report a case of cutaneous necrotic loxocelism that required surgery with favourable resolution. The goal is to raise awareness for pediatricians of the existence of this entity that it will only be diagnosed very early if we think of it, that will allow a timely and appropriate approach to the patient.
