Abstract

MORAES, Mario; DA SILVA, Lucia; FALIU, Betina  and  SOSA, Claudio. Técnica de alimentación a pecho y aparición de trauma del pezón previo al alta hospitalaria. Arch. Pediatr. Urug. [online]. 2011, vol.82, n.1, pp.10-17. ISSN 1688-1249.

Background: pain from nursing and the cracks in the nipple is reported frequently during breast feeding. It is usually referred to causal relationship between the nursing technique and trauma to the nipple. Objective: description and analysis of the technique of breast feeding and the presence of cracks in the nipple prior to hospital discharge. Materials and methods: a study of cross-sectional, prospective cut was conducted from October 1, 2009 to January 31, 2010 with a population of mothers who were in joint accommodation prior to discharge from hospital with healthy newborn babies with more than 38 weeks of gestational age and more than 2500 g birth weight. Results: 204 mothers and babies were included. The average maternal age was 24.0 years (6.63). The gestational age of the newborn infants was 39 weeks (from 1). Weight at birth was 3024 g (of 380). 114 were male newborn infants (56%). The frequency of complications was 76.5%, with 57.3% of nursing pain and 40.1 per cent from cracks in the nipple. We found an association between changes in the breastfeeding position with a 4.78 OR (95 2.22-10.29%) for a technical OR of 7.22 and one or two negative parameters (95 2.34-22.24%) for the presence of 3 or more parameters including alterations in nipple made by the baby. Having cracks in previous pregnancies was associated with the appearance of cracks in the current pregnancy with a 3.91 OR (95% 1.66-9.35). Having previous children reduces the occurrence of complications OR of 0.31 (95% 0.13-0.77). Conclusions: the frequency of breastfeeding complications is high prior to hospital discharge, and is associated with primiparity. Association between changes in the breastfeeding position was observed; difficulties in the baby taking the nipple and suction with the appearance of cracks in the nipple. The history of cracks in the nipple in previous pregnancies is associated with cracks in the current pregnancy
