The case of a five year old girl that is admitted with 2 days of disuria, abdominal pain, dark red urine and red eye is presented. The exams showed hematuria and a high calciuria. The compromise of small and large joints along with fever appeared in the evolution. The diagnosis is Arthritis Rheumatoid Juvenile (ARJ). After receiving non-steroids antinflamatories and corticoids treatment the patient's health improved and she is symptom free since then. It is noted the presence of gross hematuria and hypercalciuria as an unusual presentation of the illness.
