Summary Persistent pulmonary hypertension of the newborn, is a major cause, of high morbidity and mortality in the neonatal period. Nowadays the inhaled nitric oxide continues to be an excellent therapeutic resource, and it is the first option in treating the persistent pulmonary hypertension of the newborn. However upon not counting with this resource in the majority of the neonatal centers of our country, transfers of extremely hazardous patients are often carried out by the severity of its pathology, what leads to therapeutic delays and high health costs. In search for new therapeutic alternatives, and on the basis of very recent experiences presented in the Society of Pediatrics Research March 2004; the experience carried out in a private Intensive Neonatal Center of our country is analyzed. A serious clinical case of a newborn is presented, with severe pulmonary hypertension, secondary to a syndrome of meconium aspiration. Treatment is carried out with sildenafin oral route, pulmonary vasodilator through the inhibition of the fosfodiestarasa 5. Rapid decline of ventilation parameters is achieved, with maintenance of the systemic blood pressure and later extubation. Hospital discharge is granted at 25 days of life with normal echocardiogram.
