Summary Objective: in the first hours, after the surgical correction of the transposition of the great arteries, frequently occurs a fall in cardiac output. To diminish their impact, in this group of high risk neonates, we reported our preliminary clinical experience with the prophylactic use of milrinone. Materials and methods: six consecutive neonates undergoing surgical correction for transposition of the great arteries, received intravenous median-dose (0,54 µg/kg/min) without previous loading dose before weaning from cardiopulmonary bypass. We evaluated hospital mortality, clinical and hemodynamics parameters of low cardiac output and the incidence of adverse events due to milrinone infusion. Results: no patients died, and no low cardiac output syndrome was detected. No collaterals effects due to milrinone were observed. Conclusions: our milrinone protocol has been shown effective and safe to prevent low cardiac output, in 6 neonates undergoing surgical correction for transposition of the great arteries.
