Summary Introduction: in the context of the project "Technological transfer Canada-Uruguay" for the descentralization of the health care system and reinforcement of management capacity of hospitals in Uruguay, a task force was created within the Paediatric Hospital "C.H. Pereira Rossell" with the purpose of elaborating a clinical practice guideline (GPC). The subject selected was "first time, non-stimulated, seizure in absence of fever". Objective: unify the diagnostic, testing and treatment criteria in order to improve the quality of health care and rationalize the expenditure of resources. Methodology: the task force began work in December 2000, completing this first GPC on November 2002. This Guideline was developed in agreement with international directives for the preparation of GPCs. Stages: 1) Planning and elaboration; 2) Implementation, communication and introduction into practice. Results: in this GPC concepts and recommendations based on international consensus and a bibliographic review were included. The succesive steps to diagnosis, testing, treatment and follow-up of children presenting a fist-time seizure in absence of fever are detailed. These steps are summarized in an algorithm. Comments: the steps for planning, elaborating, validating and internal diffusion of the GPC have been completed; the publication and aplication fase is still in process. Evaluation will be performed six months after implementation.
