Abstract

Raquicentesis en emergencia. Arch. Pediatr. Urug. [online]. 2003, vol.74, n.2, pp.128-132. ISSN 1688-1249.

Summary The lumbar puncture (LP) is a frequently handling in the Emergency Department, wich prove is essential to the diagnosis and treatment of many disorders. This handling show many troubles to determine the sample contaminated with blood. In the months previous to this trial the incidence of traumatic PL was 50,6%. Craig and cols. using the depth of insertion and heigh might increase the chance of successful LP. Objective: perform LP technique to reduce the chance of obtaining a sample contaminated with blood. Material and method: 27 children that consulted required LP between 01/04/02 and 15/07/02 were included. Perform LP in either the lateral recumbent or sitting position. We use Midazolam i/v and topical cream EMLA for analgesia and sedation depend on Infant age. Make use of the spinal needle stylet. Depth of needle insertion and height were linearly related. Mean depth of insertion(cm)0 = Height(cm) x 0,03. Consider traumatic LP a sample contaminated with blood without amount of red corpuscle Results: Traumatic LP was correlated with use of analgesia and sedation, position, and experience from the member of the pediatric team. Age rank were between 15 days to 14 years. Nine traumatic LP were found (33,3%). Conclusions: To decrease the rate of traumatic PL we must realize the acute LP technique we hope that using the depth of insertion and height might increase the chance of successful LP
