Abstract

Summary It is known the effectiveness of adenosine in the treatment of paroxysmal supraventricular tachycardia showing no signs of hemodynamic compromise. It is proposed to analyse the response to this drug in children who consulted for this reason in a pediatric emergency department, regardless their hemodynamic status. It was a retrospective study of nineteen clinic histories of children whose ages range from 21 days to 14 years old, and who presented a total of 25 episodes of paroxysmal supraventricular tachycardia in a period of 5 years and 7 months. Two groups of patients were considered: one of them showing no signs of hemodynamic compromise (40%) and another one showing signs of hemodynamic compromise (60%). The return to a sinus rhythm in the ECG after a maximum of three doses in the same episode, was considered as a positive response to the adenosine. In the group showing no signs of hemodynamic compromise, in the 10 treated episodes (7 children) 70% returned to a sinus rhythm after one to three doses of adenosine. In the group showing signs of hemodynamic compromise, in the 15 treated episodes (13 patients), 93% returned to a sinus rhythm, after one to two doses of adenosine. Global response positive was 84%. No death, no side effects, and no other complications were observed. The treatment with adenosine in paroxysmal supraventricular tachycardia in the emergency department is effective in the two groups considered, and comes up as a therapeutic alternative, even in patients showing signs of hemodynamic compromise
