Abstract

To evaluate the performance of a unit of inhalatory therapy of a Pediatric Emergency Department and the efficacy of a therapeutic diagram, 200 children less than two years old with brochoobstructive syndrome were studied prospectively during the months of winter. The patients with a Tal Score of 5 to 8 were admitted in the unit to receive inhalatory salbutamol (2 puff every 20 min) during 1 or 2 hours depending on the therapeutic response; oral prednisone (1 mg/kg) was associated to the children with a previous history of brochoobstruction. The average stay in the unit was 2 hours 20 minutes; 137 children (70%) required 2 hours of treatment under brochodilators and 27 patients were additionally treated with corticoids. 54,5% of the patients from the unit were sent to their homes and 43% of the 77 children with persistent SBOL from the urgency general area. It is concluded that the unit was efficient to alleviate the Pediatric Emergency Department from the excessive demand due to SBOL in the months of winter and that the treatment with bronchodilators allowed a derivation to the children’s homes after a short hospitalization (54%) or a continuation of the treatment in emergency of the remaining 17%.
