Ischemic stroke is a common pathological entity with a high morbidity and mortality burden worldwide. Approximately one-third of events are explained by embolisms of cardiac and aortic origin. The fundamental objective of the medical team is to identify potential sources of embolisms, given that treatment differs according to the diagnosed underlying condition. In this context, cardiovascular imaging techniques play a central role, and within them, echocardiography plays a fundamental role. Patent foramen ovale is particularly important, a congenital anomaly with a high prevalence of nearly 25% in the general population, increasingly associated with cerebrovascular accidents, with paradoxical embolism being considered the main mechanism. We present the clinical case of a 76-year-old female patient who concomitantly presented a cerebral infarction and a pulmonary embolism in the context of a patent foramen ovale identified by transesophageal echocardiography.
