SUMMARY The clinical case of a 60 years old male with a history of liver transplant, with an acute inferior myocardial infarction, complicated with shock and complete atrioventricular block, is presented. We describe the therapeutic strategy applied in this case, which included a primary coronary angioplasty of the right coronary artery. The current therapeutic recommendations for this kind of patients are reviewed. After a follow up of 6 and 12 months the patient is still alive with a good functional class. This is the first case of this kind reported in Uruguay.
