Abstract

SUMMARY We report the outcome, complications and approach in 60 patients submitted to arterial switch surgery for D-transposition of the great arteries, after hospital discharge. We performed a mean follow-up of 5 years and 6 months (age range: 4 months to 11 years old) in a group of patients with these characteristics at admission time: 60% male, 25% with weight inferior to 3 kg (minimum: 1.830 g). All presented hypoxemia and 25% also showed symptoms of low cardiac output. 2/3 represented simple D-TGA cases and 1/3 complex D-TGA cases (all showed interventricular communication) with aortic coarctation and aortic arch hypoplasia in one case, mild pulmonary stenosis in one case and situs inversus in one case. 76% of the cases presented a Leiden type A1 coronary distribution, 15% presented type A2 and 9% of other types. All received E1 Prostaglandin and Rashkind. Nitric oxid administration was necessary in four cases in order to reach an appropriate saturation level. All were submitted to arterial switch surgery. After discharge, we performed both a clinical and a paraclinical follow-up through ECG, color doppler ultrasound, Holter and Ergometry studies. 80% showed a positive evolution. In this group, trivial residual damage prevailed (pulmonary or aortic supravalvular stenosis, aortic, pulmonary, mitral or tricuspid insufficiency). Nine patients (15%) required additional interventions: pulmonary artery plasty in seven cases (11,6%), in one case surgery for residual interatrial communication surgery was needed, in another case, resection of subaortic stenosis was performed. Three patients died (5%) with dilated cardiomyopathy; one of them having underwent a pacemaker implantation, the other having underwent mitral valve replacement. This study shows, just as stated by the literature on the matter, that medium-term evolution of patients submitted to anatomic correction of D-TGA is good. Supravalvular pulmonary stenosis is the most frequent cause of reintervention.
